An amnesiac milkman charged with traveling between walled cities to make deliveries.
- Badass Driver: Comes with being a delivery guy in a post-apocalyptic world. He's so good that Raven picks him to compete in Calypso's tournament.
- Bunny-Ears Lawyer: He’s talkative, irreverent, and one of the best milkmen out there.
- Dub Personality Change: In the Mexican Spanish dub, John sounds more insanier and wittier, basically like Deadpool, or at least, the film version of him, as a black man, complete with a more wisecracking tone in his voice, compared with Anthony Mackie's suave voice.
- Courier: His job as milkman involves ferrying supplies between the various walled cities.
- Identity Amnesia: Remembers nothing about his life before waking up in a car as a young boy.

A less-than talkative bandit seeking vengeance for her late brother.
- Canon Foreigner: She's not based on any character in the game.
- Fingore: Was Made a Slave and had her middle finger chopped off by her boss.
- Just Like Robin Hood: In the season finale, she robs milkmen’s cargo and gives them to the squatters living outside the walls.
- Made a Slave: Took a work contract that promised her beachfront property after several years of service. This was a scam and she was treated like a slave.
- Revenge: Seeks to take down Agent Stone for his part in his brother’s death.
- The Silent Bob: Lives up to her name as it's not until late in Episode 2 that she finally speaks. After that, she can vary from back to mute to spouting out wild lines.

The COO of New San Francisco.
- Age Lift: In Twisted Metal Black, Raven is seventeen. Neve Campbell was 49 at the time the show was released.
- Adaptation Personality Change: In the games, Raven is a surly rebellious Goth teenager. Here, Raven is a mature and manipulative politician.
- Manipulative Bitch: Assembles a fake family to emotionally manipulate John into taking her job. Later has several spies befriend him and has one of her police officers date him.
- Villain in a White Suit: She wears a white dress when she first meets John. She changes to black when she reveals her true agenda at the end of Season 1.

A large psychotic killer in a clown mask who is also the ruler of Lost Vegas.
- Acrofatic: He's a lot quicker and more agile than his heavy build makes him seem. Being played by a wrestler certainly helps.
- Adaptational Comic Relief: He's a hammy showman in the series who is the source of most of the laughs outside of John's wisecracks, unlike the games where he's a full-blown mass murderer obsessed only with killing.
- Adaptational Curves: Zigzagged, his iconic design didn't appear until Twisted Metal Black, up to that point he mainly appeared to have a skinny or slender appearance. In the series, Sweet Tooth appears to have a burly build, appearing obese but, nevertheless, still muscular.
- Adaptational Nice Guy: He's a lot more legitimately chummy and affable here than he was in the games, even if he's still an insane serial killer. How he ended up institutionalized is also changed to be comparatively less serious and more pitiable. Rather than being sent to the asylum for murdering his whole family, he was sent there for snapping and killing his dog co-star on the set of a sitcom his parents pressured him into auditioning for.
- Adapted Out: His Split Personality has been completely excised from the series.
- Affably Evil: He's a sincerely friendly, jovial, and witty guy who's rather fun to hang out with. Unfortunately, he's also a deranged, homicidal Serial Killer who loves to kill with abandon.
- Berserk Button: Don't lie to him. He can take honest criticism, but he has no tolerance for lies.
- Big Bad Ensemble: With Agent Stone.
- Calling Your Attacks: He'll say the name of whatever weapon he's throwing whether it's "Machete!" or, in one case, "Woman's body!"
- Can't Take Criticism: Subverted. He kidnaps people and forces them to watch his show but he only kills those who don't give him honest feedback. He spares Quiet and John after they tell him his show sucks and convince him to leave the hotel to gain new experiences to base his play on.
- Celebrity Survivor: He was a Former Child Star before the apocalypse, having snapped on the set of his show and killed his dog co-star in front of a live studio audience in an incident that got him institutionalized. Now, he's a Large Ham showman driven by a desire to become famous again.
- Companion Cube: His best friend is Harold, a paper bag.
- Composite Character: His appearance and name, Marcus Kane is lifted from the 2012 game, but his Large Ham personality is more in-line with his characterization from the original PS1 games, particularly Twisted Metal 4.
- Cop Hater: Due to being a serial killer. With the help of Stu and other prisoners he's freed, he attacks Stone's followers.
- Does Not Like Spam: After rescuing Stu and having a chat with the guy, Stu ends up finding a box of Mike and Ike and offers some to Sweet Tooth. Sweet Tooth declines with an "Ooh, yucky" before saying he's a Skittles guy.
- The Faceless: At least as an adult, he has yet to be seen without his mask.
- Flaming Hair: Covers his scalp in a flammable substance and lights it during the final battle of Season 1.
- Former Child Star: His origin story. He was pressured to join a sitcom by his parents where he was overshadowed by his dog sidekick, Billy. He eventually snaps and murders the dog in front of a studio audience. His parents sent him to Blackfield Asylum and refused any more communication with him. In fact, Billy was so much more popular than Marcus that the dog's funeral received higher ratings than his own trial.
- Freudian Excuse: He had abusive parents who forced him to perform alongside a dog on a TV show, where he was constantly humiliated and upstaged by his co-star. Reaching his breaking point, he furiously stabbed the dog to death with sewing needles in front of a live audience. Once he was incarcerated, his family cut off all contact with him despite his attempts to reconnect with them, making him realize that they never cared for him.
- Hidden Depths: He's familiar with the works and advice of Mark Twain and is humble enough to accept criticism, only murdering people for lying to him by saying his work is good. In fact, he comes to the conclusion that he should leave the casino and explore the world so he can improve his craft instead of limiting himself by staying in the hotel.
- I Have Many Names: He goes by "Sweet Tooth" but his original name is Marcus Kane and he was nicknamed "Needles" Kane after killing his dog costar with a pair of knitting needles.
- Iconic Attribute Adoption Moment: In the Season 1 finale, he has Stu set the top of his head ablaze, finally giving him the Flaming Hair from the games.
- Kevlard: He's a heavyset man but can take a lot of damage and deal just as much.
- Large Ham: Has the personality to match his larger than life showman persona.
- Made of Iron: He is incredibly durable as he's able to withstand several punches in the series. He also gets shot by Stu and hit by a car but survives without issue.
- Monster Clown: A serial killer who always wears a clown mask.
- Photographic Memory: At least, he claims that he can remember everything, including his own birth.
- Pragmatic Adaptation: Due to the Split Personality aspect of his character from the games being Adapted Out, Sweet Tooth's real name is Marcus Kane, while his official name from the games, Needles, has been relegated to a nickname bestowed upon him by the media due to him murdering his dog co-star with knitting needles.
- Real Men Wear Pink: He's a fan of N Sync, crossdresses in his show, and is an excellent cook.
- Sinister Sweet Tooth: It's not just in the name. His vehicle of choice is an ice cream truck and he shows a fondness for Skittles. His former career as a child star also starred him as a kid working in an ice cream shop.
- Stout Strength: He's able to fight John effortlessly and is able to decapitate several people in one swing.
- Trademark Favorite Food: Skittles.

A tyrannical cop looking to restore law and order to the fallen nation from Topeka, Kansas.
- Adaptational Villainy: Depending on the timeline, he's either a James Bond expy or a Shell-Shocked Veteran who wants to undo his greatest failure in the games, but here, he's a fascist warlord that isn't heroic in any way.
- All Crimes Are Equal: His regime in Topeka treats something like failure to curb one’s dog with the same severity as murder, owing to his belief that small sins attract bigger sins.
- Ancestral Weapon: During the apocalypse, his neighbors stole his service pistol so he took his father's old 1911 pistol. He still carries it by the time of the series.
- Big Bad Ensemble: With Sweet Tooth.
- Fake Ultimate Hero: Mike learns that Agent Stone rescued his hometown from an army of terrorists during the apocalypse. In reality, the "terrorists" were just a few ordinary citizens raiding a restaurant for food. The insecure Stone killed them all, including some civilians, after they mocked him for being a mall cop.
- Faux Affably Evil: Stone is polite, seemingly down-to-earth, and friendly in a stern sort of way. It's all a mask for how much of a ruthless, power-crazed tyrant he really is.
- From Nobody to Nightmare: Started off as a beat cop who was so low-ranking and disrespected that he was mistaken for a mall cop. By the time of the series, he's basically a warlord running a fascist state.
- Not So Stoic: He usually depicts himself as a tough, masculine lawman but he's nearly on the verge of tears when he repeatedly fails to stop Sweet Tooth.
- Scars Are Forever: He has a scar on his nose, gained from being hit by a rifle butt shortly after the apocalypse began.
- Villains Out Shopping: He likes collecting state quarters and a Word of the Day calendar that he actually uses in his Evil Gloating.

Agent Stone's Lieutenant.
- Adaptational Villainy: In the games, Shepard is one of if not the most heroic drivers, immediately seizing his opportunity to arrest Calypso the second he wins. In the show, he's the sadistic lieutenant of the fascist Agent Stone as well as a pedophile.
- Pædo Hunt: Shepard is caught masturbating to a lolicon manga right before Quiet kills him.
- Screams Like a Little Girl: He lets out a noticeably high pitched scream before Quiet kills him.
- Undignified Death: He's beaten to death with a gun after being interrupted pleasuring himself to a lolicon manga.

Two former guards who were exiled from their town after Stu was caught sleeping on the job. They join Agent Stone's police force. Stu later leaves to join Sweet Tooth.
- Professional Butt-Kisser: When Stu is rescued by Sweet Tooth and forced to be his right-hand man.
- Sensitive Guy and Manly Man: Mike easily passes the test Stone sets up for them while Stu fails. He eventually betrays Stone when he doesn't have the strength to execute John and Quiet.

A Bumbling Henchmen Duo working for Stone.
- Adaptational Villainy: Carl and Jamie Roberts, the original two drivers of Outlaw, were some of the more sympathetic characters of the games. In the show they are Butt Monkeys and a Bumbling Henchmen Duo working for Stone.
- Brother–Sister Team: They're siblings and frequently work together.
- Butt-Monkey: They're basically a Bumbling Henchmen Duo and the idiots surrounding Stone. They spent their last scene in season 1 in neck braces and eventually get killed.

A race car driver and the leader of the Convoy. Goes by the codename "Twister".
- Butch Lesbian: A Wrench Wench queer woman who doesn't shave her armpits. She's in a relationship with Flower Power.
- Hairy Girl: Leaves her armpits unshaved.
- Masculine–Feminine Gay Couple: Less traditionally feminine than her girlfriend Amber.
- You Are in Command Now: In her first appearance, she is the second-in-command to Granny Dread, who dies at the end of the episode.

A botanist and the ex-girlfriend of Miranda Watts. Goes by the codename "Flower Power."
- Caring Gardener: She grows plants for a variety of purposes including food, healing, poison, and beauty.
- Granola Girl: A flower-loving hippie who lives in the country.
- Healing Herb: One of her products.
- Masculine–Feminine Gay Couple: She's much more traditionally feminine than her girlfriend Miranda.
- Psycho Lesbian: A rare sympathetic example. She kills people and uses them as fertilizer but they're Asshole Victims and spares John and Quiet once she realizes they work for Granny Dread. Her breakup with Miranda is a Noodle Incident. All that's known is Miranda pushed her out of a truck after Amber poisoned her.

Cathy Stropton, an elderly woman and the leader of the convoy.
- Adaptational Badass: In her home game, her status as a badass granny is an Informed Attribute beyond her participation in Twisted Metal. Here, she's the former leader of The Convoy and certainly earned the nickname of "Dread".
- From Nobody to Nightmare: Granny Dread was originally a real estate agent before becoming the feared leader of the convoy.
- King on His Deathbed: She's the leader of the Convoy, a Mobile City of big rigs. In her first appearance, she is near death and leaves Miranda in charge.
- Never Mess with Granny: According to Miranda's eulogy, she was so feared she earned the nickname "Granny Dread."
- Shipper on Deck: Her last words to Miranda are encouraging her to get back together with Amber.
- Viking Funeral: Her body is put in her car, pushed out the back of a trailer, and blown up.
- Portrayed by: Chloe Fineman
A milkman and a former owner of a Pits (a milkman truck stop).
- Amicable Exes: With John. When her Pits was burned down, she was hurt he didn't look for her because he assumed she was dead but otherwise doesn't hold a grudge. She eventually betrays him, but out of greed rather than anger.
- Cute But Psycho: While she's a far cry from her video game counterpart, she has no problem sneaking banned weapons into a Pits, picking fights with men much larger than her, urinating out in the open, or betraying John and stealing his package.
- Hated Hometown: Originally worked at a Pits owned by her mother. Would have inherited it but it was attacked and burned down, something she grew to be grateful for. Became a milkman herself afterwards.
- The Missus and the Ex: Meets and befriends Quiet.

A former minister who now runs a hedonistic cult known as the "Holy Men".
- Adaptational Villainy: In his home game, he was a delusional preacher who genuinely believed he was demonically possessed during his murder spree, and is Driven to Suicide upon finding out the truth. Here, he's the leader of a hedonistic cult.
- A God Am I: A former Christian minister who became disillusioned after the apocalypse and now believes that humans are the real gods.
- Badass Preacher: Put up quite the fight against John Doe.
- Bondage Is Bad: Has several dominatrixes in his gang.
- Hollywood Atheist: Believes humans are the closest thing to gods there are. Also steals, tortures, kills, does drugs, and practices BDSM.
- Portrayed by: Creek Wilson
A bartender at the Pits.
- Adaptational Personality Change: Mr. Slam goes from an arrogant architect to a friendly bartender. He's also the only milkman at the Pits who doesn't turn on John and Quiet when he finds out how valuable their package is.
- The Bartender: Runs the bar at his Pits. Also dispenses some luxury items and teaches Quiet about milkman culture.
- Gag Penis: Entertains patrons by smashing watermelons with his giant penis, which is the source of his nickname.
Calypso
John's contact in New Chicago. He's also preparing a tournament of the best drivers around.
- The Faceless: In his only physical appearance in Season 1, he's shot only from behind.
- The Voice: In Season 1, he only communicates through a speaker.
Dollface
A woman in a doll mask claiming to be John Doe’s sister. She leads a gang of women in similar masks.
- Last Episode, New Character: She appears to confront Quiet in the season one finale.
- Related in the Adaptation: She claims to be John Doe’s sister.
- Screaming Warrior: Her gang let out a high-pitched scream when they surround Quiet.
