Too often this is used as shorthand for unplanned pregnancy or But I Can't Be Pregnant! rather than a late realization. On the main page, most of the examples are relevant, but on other pages misuse is rampant. I've done a random wick check of 50 links (with the context descriptions) and the results aren't pretty. This is the first time I've done a wick check so idk if I formatted it right.

    Examples 

Correct use 1. Like Father, Like Daughter: Both Marco and Hekapoo are surprised at Hekapoo's pregnancy as neither thought it was possible for them to have a child together. Hekapoo even states that she didn't even know she was pregnant until she went to the hospital to find out why she felt 'sick,' only to find herself going into labor right when she arrived. Janna doesn’t buy Hekapoo’s story and insists she was lying about thinking Marco couldn't impregnate her, noting that a boy like him WOULD use protection. She later confronts Hekapoo about this when in a room that forces everyone to tell the truth, and it's revealed that Hekapoo was being honest.
2. Aunt Dimity: Lori is so busy tracking down Willis Sr. in Aunt Dimity's Good Deed that she fails to appreciate the significance of her cravings and morning sickness until Willis Sr. relays the phone message he got from her doctor.
3. Dancing on the Edge: It's mentioned that Miracle's mother's pregnancy barely showed at all even in the later months, and her entire stomach was at most the size of a basketball.
4. Ana Satsujin: It's revealed in the final chapters that while in a coma, Miyaichi was pregnant with her and Kurosu's child, one that they sadly only learn about in their final moments.
5. Fringe S01 E02 "The Same Old Story": Given that it's a rapidly-aging and growing baby, yeah.
6. Warrior Cats: Bluestar's Prophecy: Bluefur is unaware that she is pregnant until another she-cat points it out.
7. FTL Faster Than Light Captains Edition: One positive event when socializing with Mantis crewmember has them suddenly reveal to be pregnant and going into labor, before giving birth to a full-grown Mantis ready to serve.
Misused 1. JMS' Spider-Man: In Sins Past, Gwen Stacy discovers she's pregnant from her affair with Norman Osborn and gives birth to twins 4 months early.
2. Nemi: A 2017 strip has Cyan finding out she's pregnant after a one-night stand in Prague
3. Salma Hayek: In Fools Rush In, a one-night stand with Matthew Perry leaves her pregnant.
4. Us and Them: Aeris and Seph had intended to wait before having children, but Aeris misread the instructions on the birth control and wound up getting pregnant anyway.
5. Conversations with a Cryptid: All for One was shocked that he was virile enough to sire children when he accidentally impregnated Inko with Izuku. It turned out to be a result of his vitality quirk.
6. Extended Stay: The Mistress and the Superjail crew find out she's pregnant just when Ultraprison is about to leave. This one is justified since she hasn't been far enough along to exhibit any symptoms.
7. Family Matters: Well, it's a "surprise" in that they weren't expecting to have a kid so soon after they got married—Ash and Misty had only been married for about four months when Misty discovered she was pregnant with Aiden and Michelle while May and Drew had only been married for about a month by the time she found out that she was pregnant with Damien. Ash and Misty were especially shocked to discover that they were going to have twins instead of just one kid. 8. Five Score, Divided by Four: Dave fears one of these after a nightmare, only to be reassured she isn't by Fiona.
9. Red Fire, Red Planet: A one-shot MACO named Master Chief Special Warfare Operator Haellh mentions to Admiral Riker that she's six weeks pregnant. Another MACO, Petty Officer Freeman, complains about her not saying anything, but Haellh says she herself only found out a few hours earlier.
10. The Second Try: Due to her mess of insecurities, Asuka flipped out when she discovered she was pregnant and had tried to hide it from Shinji (She even went as far as to starve herself in a misguided attempt to induce a miscarriage). He only found out after a rat got into their trash and he stumbled onto a positive pregnancy test Asuka tried to hide. To say he was surprised would have been an understatement. Incidentally, Asuka avoids the usual markings of trope, as what tipped her off to her condition was missing one of her periods.
11. Unexpected Surprise: After one night together.
12. China White: Ann discovers she’s pregnant with Bobby’s son, but only after Bobby’s death. She eventually flees all alone to Brazil to raise his son after the end of the film.
13. Tyson: Played with. Robin fakes her pregnancy so that she can get married to Tyson immediately, and uses it to engender sympathy from him and his associates.
14. A Boy Made of Blocks: Sam was conceived one drunken night during a cheap package holiday in Spain a year after Alex and Jody met.
15. Breathing Underwater: Nick was "a mistake" that happened because his mother didn't use enough protection.
16. Ixia and Sitia: Yelena discovers she's pregnant at the end of Shadow Study after realizing she forgot to take her birth control potion.
17. Just as Long as We're Together: Gena is going to have a boy, which makes Allison worried she will be replaced.
18. Summer Sisters: Victoria's sister Lanie gets pregnant when she's only seventeen.
19. The Blood War Trilogy: The Princess' one by Arrin results in his becoming The Exile.
20. The Devil Wears Prada: Emily is shocked to learn she's pregnant.
21. The Princess Diaries: Mia is pregnant with twins!
22. Tortall: A Spy's Guide: Alanna, who is in her late forties in 466 H.E., writes George in October of that year to reveal that she's pregnant as a result of a damaged anti-pregnancy charm and them spending time together in May. This is the impetus for both George recovering the documents that form the book while cleaning a room and Alanna's retirement as King's Champion.
23. Manga/IO: It's implied Mar is pregnant with Taiyo's child, as when Machiko activates the temple she feels pain and heat in the vicinity of her uterus.
24. Sekai Oni: Before the third World Devil, Kuroe realizes her latest fling gave her this. The result is an absolute mental breakdown and demanding sex out of every single man present at the next fight, and upon considering that she could use her unborn child as a reserve tank, snaps into a Manifestation of lust that endangers the entire cast. Surprisingly, the story actually confronts this afterwards, as Kuroe's eventual resolve to keep the child makes her stable again.
25. Eric Church: Averted in "Two Pink Lines". The young couple in the song is worried because the woman is two weeks late. At the end of the song, the test comes up negative, and she leaves.
26. Koda Kumi: She announced that she was pregnant a few days after she said she was getting married to BACK-ON's Kenji 03. It was probably a Shotgun Wedding.
27. Babylon Five S 05 E 19 The Wheel Of Fire: Delenn suddenly faints and when Dr. Franklin examines her, he determines she's expecting. Sheridan is quite surprised — and then thrilled.
28. Castle S 5 E 12 Death Gone Crazy: Candance Armstrong had slept with Beau causing her pregnancy.
29. Gilmore Girls S 03 E 16: Sookie is pretty oblivious to her symptoms, so it's doubtful she was trying to get pregnant.
30. Law & Order: Special Victims Unit S8 E22 "Screwed": Kathy reveals to Elliot that she's pregnant. As they're in their forties, Elliot is understandably shocked.
31. Lucifer (2016) S04E02 "Somebody's Been Reading Dante's Inferno": Linda is revealed to be pregnant with Amenadiel's child.
32. Orange Is The New Black S 05 E 06 Flaming Hot Cheetos: Lorna realized she was pregnant during her lovemaking session with Nicky when she saw her nipples were a shade darker, a trait common to Morello women.
33. Once Upon a Time S3 E12 "New York City Serenade": Variation—the pregnancy was both planned and known about by everyone when it happened, but thanks to the curse taking everyone's memories of the last year, from their perspective it is now the next day after Emma and Henry left Storybrooke and Mary Margaret is suddenly pregnant.
34. Timeless S2E5 "The Kennedy Curse": Apparently, Carol was telling Lucy the truth that she wasn't planned (even though her relationship with Cahill probably was). In fact, Carol planned to give her up for adoption in order to keep her away from Rittenhouse.
35. Love Keeps Going: Yi Feng and Yi Lei's sister Yi Fei. Her lover took off as soon as she was pregnant.
36: The Jury: Katherine's arc in the second season. A secondary school teacher, Katherine was having sex wtih a seventeen year old but fell pregnant, which really shouldn't come as a surprise.
37. Betrayal: Almost certainly Emma's with Ned; given the parlous state of her marriage and the fact that it ruins her relationship with Jerry it seems likely that it was accidental. This is interesting since the play comments on the more open and casual attitudes to sexuality in the 1970s, which developed in large part from the introduction of the Pill in the 1960s. Showing through the pregnancy that sex is still not without obvious consequences therefore underscores the more subtly depicted repercussions of the affair and the message that Everybody Has Lots of Sex is not really making people any happier.
38: If/Then: In both timelines; Liz with Josh and Beth with Lucas.
Lacking context 1. Mobile Suit Gundam Gran Tomino: Sayla.
2. Solar Winds: Quinn, just as in Glee. Unlike Glee, however, the father is not yet known.
3. Sex and the City: Charlotte.
4. Soul Food: For Bird and at the worst possible time.
5. Daddy's Daughters: For Ludmila and Dasha.

Conclusion: 14% correct use 76% incorrect use 10% lacking sufficient context

I'd say these results speak for themselves. The fact that the main page has mostly correct usage suggests the name itself is probably the most misleading. I think a better name would be Late Pregnancy Realization.

That might be a mistake on my part. I haven't read that storyline, but I happen to know that that pregnancy progressed unnaturally fast leaving her full-term at five months, but it doesn't say she only then realized she was pregnant.

Yes to the rename, I misunderstood this trope for years because of all the misuse.

I also suggested this as a page image in the Image Suggestion Thread to make the trope more obvious (maybe cropping out the ad part).

There are two options:

  • A moderator can create the page with the new name.

  • Use TLP; create a draft with the new name and launch it. Surprise Pregnancy then gets dewicked and the new name cross-wicked. The old page gets cutlisted, or if the number of inbounds is too high, it become a redirect (but that attracts misuse, as people keep using the old name because it appears as a blue link).

Maybe it's a newer trend. Because people lately have been turning against the idea of sending these pages back to TLP unless necessary, and name changes that don't change the definition don't need TLP.

[up] We mainly use TLP for proposing and launching new tropes. :-)

The thing is, only mods can create new articles in the Main/ namespace. And they are likely busy and overworked, so they usually leave this work for regular editors. (It was the case with TRS of Agony of the Feet and Hurt Foot Hop. Everything was prepared in the TRS thred: new name was picked and there was an approved draft description. Everyone in the thread okayed it and we asked about launching it with hollers and in ATT, yet mods didn't choose to launch it and instead, we had to make the TLP.)

This was I think a year or two ago, so perhaps things have changed again. But I see no mods in this discussion announcing their intention to launch the new page.

I suppose another possibility would be to holler the latest posts, or better yet, to post in the Locked Articles thread (which is also for creating redirects).

I think this depends on the OP and how they handle it with the mod team.
