That's UB-10 Lich, an Ultra Beast I made up for a fanfiction I'm writing. It's based on Schrödinger's Cat. What look like its eyes are actually just holes - it sees using a photoreceptive lining in the back of its head. Here is the specific drawing that avatar is from.
