- The ABCs of Death (2012) note Extensive self mutilation by buzz saw, child's head gorily smashed, bound victim chainsawed limb by limb, sodomy by drill, bound naked woman brutally gutted alive, and much more.
- ABCs of Death 2 (2014) note Different but similar stuff to above.
- The Accused (1988) note For the extremely graphic and prolonged gang rape sequence. One of the most graphic for a mainstream legal drama.
- Adam Chaplin (2011) note Extremely brutal, gory dismemberment, disembowelment, blood-spraying, beheadings and a chained woman badly burned.
- Aftermath (1994) note A man rapes a woman's corpse by thrusting into its open chest cavity (his buttocks are visible). Very extensive and graphic autopsy gore throughout.
- Aftershock (2012) note For Gringo and Irina's deaths. The former is caught under heavy rubble, is brutally tortured by thugs and forced to watch the latter get brutally raped at length by several thugs before she is bloodily shot as she runs away; with the former then being hideously burned alive. Other hard 8 violence throughout.
- American Mary (2012) note For Grant's hideous/torturous surgical mutilations. Otherwise level 8.
- Angst (1983) note Young woman brutally tortured, killed, and implied to be raped afterward. Other cruel but non-gory violence includes a restrained elderly woman slowly dying by heart attack, and a mentally and physically disabled man drowned in a bathtub.
- AntiChrist (2009) note Due to the self-inflicted genital mutilations before the end
- The Anthropophagus Beast (1980) note Pregnant woman has her fetus ripped out of her and eaten onscreen; man is disemboweled and he eats his own guts.
- Antlers (2021) note Horrifically mangled bodies viewed in extreme detail. One is seen having their fingers, nose, and throat bitten out before having their heart removed offscreen.
- A Zed and Two Noughts (1985) note For the unsimulated decaying of animal carcasses. Otherwise, Level 5.
- À l'intérieur (2007) note Repeated; extremely vicious, brutal and gory stabbing (eyes, groins, necks; often all at once) and gory ceasarian and killing of preganant woman.
- Alien 2: On Earth (1980) note Childs face mangled unrecognizably, impecably detailed and lingering shots of gore.
- Aliens vs. Predator: Requiem (2007) note Due to the Predalien's impregnation of the pregnant women. Otherwise, sits on the range 8-9.
- Army of the Dead (2021) note Mainly for the zombie that gets blasted to gory pulp piece by piece in slo-mo by a .50-cal turret in the opening montage, another zombie blasted by mini-turrets before being smashed by a crusher and then slowly torn apart as it retracts; and Martin's death. Hard 9 violence throughout includes countless gory headshots among other decapitations and shootings, gruesome flesh and neck tearing, eviscerations, and ludicrous gibs by explosives.
- The Autopsy of Jane Doe (2016) note Although the most graphic gore is not aggressive in nature, the presentation is extensive, detailed, and in your face, enough to reach this level. The story also has a very disturbing tone and implications.
- Backcountry (2014) note Probably the nastiest bear mauling put on film.
- Bad Taste (1987) note Head is gorily busted open, with lots of brains slowly gushing out as the still-live victim tries to put it all back in. Body sloppily chainsawed from head to toe from within; and much, much more.
- The Banana Splits Movie (2019) note Victim bound and gorily sawed in half with all of his guts spilling. Another is pinned down and has all of his limbs gorily peeled off while he's still alive. Victim run over and crushed by speeding car in very sloppy fashion. Other level 9 violence including face smashing, both eyes being stabbed out, etc.
- The Beyond (1981) note Man gruesomely tortured and crucified. Paralyzed victim slowly having their eyes, tongue and other facial features gruesomely removed. Other level 9 violence including a gory headshot, eye gougings, and face melting.
- Blackkklansman (2018) note For real photographs of Emmett Till's mutilated corpse at a funeral alongside other photographs and videos of real-world racist violence, including the vehicle ramming from the Unite The Right event. Otherwise level 2 for a white supremacist being non-graphically blown up by an explosion.
- Black Sheep (2007) note Insane hyper-gory carnage. Much is comedic, but the level of gore persists even in non-comedic scenes.
- Bliss (2019) note Extremely gory and graphic ripping/mutilation of flesh, unfathomable levels of disturbingly realistic gore. Heavy focus on injury detail and mutilation, with borderline sexual overtones.
- Blood Feast (1963) note Disgustingly mangled heads. Other level 9 violence including lumps of organs, very gory dismemberment, and a heart cut out and squashed.
- Blood Fest (2018) note Gory chainsaw slicing, beheadings, Saw-style gory trap, etc.
- Blood Quantum (2019) note A zombie is seen eating their newborn. Genitals eaten off. Zombie strung up by it's guts before having it's head blasted off. Two humans are gruesomely torn apart. Assorted hard 9 violence and gore throughout. All with ghastly practical effects.
- BloodRayne (2005) note For the gory massacre of villagers in the unrated version. Otherwise a hard 8-9 for gushing practical blood from swordplay, head smashing, and more.
- Body Melt (1993) note Grotesquely detailed body melting and other gory body horrors; including to pregnant women.
- Bone Tomahawk (2015) note Extremely gory and sadistic murder/mutilation of bound victim. Pregnant women shown horrifically mutilated
- The Boxer's Omen (1983) note Incredibly ridiculous and silly-looking but extreme and disgusting gore, such as a man combining a brain and snake venom into a poisonous juice in the decomposed skull, and a group of cultists stuffing a human corpse into a crocodile corpse in order to reanimated it.
- Braindead (1992, also known as Dead Alive) note Every mutilation known to man, woman, and creature; and some new ones too. Much is comedic, but it's all so transcendantly gory it doesn't matter.
- Brimstone (2017) note Mainly for a gutted lamb with it's murdered fetus shown, and Eli getting disemboweled and restrained with his own intestines.
- The Burning Moon (1992) note Old woman's fingers sliced off and then decapitated; old man's arm chopped off, face stabbed and then decapitated; teen girl has back sliced open, stabbed countless times, face smashed to pulp and then burned to crisp; old man shot countless times, young man bashed with hammer and then savagely battered to pulp on concrete before being impaled by pitchfork. None of this has anything on the unspeakable Turbo-Gorn in the finale set in hell.
- Caché (2005) note Here solely for the fully onscreen decapitation of a real chicken that even runs around without its head for several seconds. The scene is Played for Horror and shock value as opposed to a matter of fact way placing it this high. The infamous throat cutting suicide is on level 8.
- Caligula (1979) note Explicit genital mutilation. Child's head smashed. Two of the unsimulated sex scenes, depicting the rape of a virgin and Caligula shoving a fist up a man's anus, also reach this level with ease. Otherwise, a 9 for several messy decapitations, a disembowelment, and an exceptionally brutal series of stabbings.
- Candyman (1992) note For a flashback where a child is gorily castrated with blood everywhere as he screams in pain as well as a glimpse of his parts in a toilet. Otherwise an 8.
- Cannibal Holocaust (1980) note Explicit gory torture, and unsimulated animal cruelty.
- Cannibal Man (1974) note For the very graphic slaughter/bleeding of real live cattle in the beginning. Otherwise a hard 7 for a bloody throat cutting, cleaver to the face, and wrench to the head.
- Castle Freak (1995) note For Sylvia getting her breasts and genitals eaten by the freak; all while she's still alive. Other violence is on level 8 to 9.
- Cat Sick Blues (2015) note Victim getting beheaded with a shovel, Claire getting raped onscreen, a woman beheaded and her head impaled into a light, head-smashing with a porcelain cat statue, disembowelment, victims and Ted getting their throat cut with sharp steel claws, young victim beaten to death, young woman getting half-beheaded with a spiked strap-on, and Claire forced by Ted to drink the victims' blood and having a nightmare about her giving birth to a deformed human-cat hybrid baby.
- Child of God (2012) note A rotting corpse is graphically raped several times.
- City of the Living Dead (1980) note Vomiting out of all organs, and much more.
- Clockers (1995) note For the graphic, realistic, full color recreations of real-life murders in the opening montage.
- Clown (2014) note Due to most of the gory violence being directed towards children
- Cockneys vs. Zombies (2012) note Dismemberment, onsceen zombies eating people scenes, messy gunshots, zombified baby getting beheaded by a sign and Mickey reduced into a pile of gibs thanks to a grenade in his mouth. Although are Played For Black Comedy, these scenes are gory enough to earn this level.
- The Collection (2012) note Hundreds of dancers are grinded to mulch up by a collapsing cieling trap. Extreme autopsy gore. Piles of mangled festering bodies. Hostages gorily killed. Other level 9 gore.
- Come and See (1985) note For the very graphic real-life footage of Holocaust victims towards the end of the film, showing numerous dead bodies, starving people, and eyeless corpses. Otherwise an 8 for cruel, disturbing violence including civilians being trapped inside of a barn which is then set on fire, a badly burned old man, a teen girl being heavily implied to be horribly raped, and a scene of a cow being killed with some blood but not in particularly graphic fashion. Of particular note is that despite all of these acts being mostly implied, they're still pretty horrific, and would earn the film a 10 if they were shown on-screen anyway.
- Cradle of Fear (2001) note A few seconds are shown of the infamous Chechclear video; real and graphic footage of a man having his throat cut open. Fictional violence is a hard level 9 for very over-the-top gory carnage throughout.
- Crash (1996) note NOT the 2005 film of the same name. Multiple graphic depictions of car crashes and people receiving sexual pleasure from them and their resultant injuries - photographs of and people posing next to hideously mangled corpses, a notorious scene with a woman’s leg scar.
- Crimes of the Future (2022) note Multiple, prolonged scenes of graphic surgery footage and autopsies with close up focus of internal organs, tumors, and other body parts being pulled out, examined, replaced, etc.
- Cube Zero (2004) note Victim gorily melted layer by layer.
- Da 5 Bloods (2020) note Hideous war casualties both real and simulated; including civilians and children.

- Dead Snow (2009) note Very messy tearing apart of humans and zombies with practical effects. Unlike it's sequel, most of the violence is played straight.
- Deathgasm (2015) note Repeated sloppy mutilations; including genitals and "metal imagery" (defacating organs, etc).
- Death House (2018) note Hideously mutilated bodies and skinless, somewhat humanoid monsters (one eats thier own baby). Other level 9 violence and gore.
- The Devils (1971) note Victims of the plague shown in graphic close-up, sexual torture involving enemas, violent torture involving a man having his legs crushed, then burnt at the stake in explicit detail.
- The Devil's Double (2011) note For Kamel's death by getting his feet sliced off, slashed in the hands and face, disemboweled and then executed with a shot to the eye. The assorted shootings, suicides, and torture are on level 8.
- The Doom Generation (1995) note Brutal, gory castration; etc.
- Doomsday (2008) note Due to Dr. Talbot getting cannibalized by the punks. Otherwise a 9 for gory head obliteration, dismemberment and smashing of bodies.
- Downrange (2017) note Extreme, repeated, sadistic gory trauma to heads, bodies, and limbs with bullets, fire, crushing by car, and more; with lingering shots of festering gore.
- Dredd (2012) note For Ma-Ma skinning (offscreen, save for a brief flashback of it later on) 3 goons alive and splattering them from a great height (onscreen). We see them as they hit the ground and later in a crime scene with brains splattered and raw flesh exposed. Otherwise an 8-9 for graphic (sometimes slow-mo) shootings (including civilians), eye gouging, an exploding arm, a rape hallucination, a head burned from the inside out, and another shredded with SMG fire.
- Dude Bro Party Massacre 3 (2015)
- Ebola Syndrome (1996) note Almost every act of depravity and inhumanity in the book, accompanied by plenty of Gorn. Numerous bloodily and gorily depicted murders and rapes, sometimes both at once. Real-life frog and chicken killings. The main character disposes of several corpses by grinding them into human meat, which spreads Ebola throughout South Africa and Hong Kong, causing extremely graphic and disturbing scenes of the illness liquefying its victims from the inside out (albeit exaggerated).
- El Topo (1970) note Real mutilated animals throughout. Gruesomely massacred villagers at the beginning; including many children and a still living horribly mutilated one MercyKilled by the protagonist; a gory castration followed by shotgun suicide; countless graphic, brutal, and at times incredibly cruel executions.
- Eraserhead (1977) note For the explicit, drawn out dissection and gradual grisly death of the "baby". Even though it's in black and white, it's brutal enough to reach this level.
- The Evil Dead (1981) note Extremely detailed, layer by layer decomposition and exploding of humanoid monsters.
- Evil Dead (2013) note Victim has their face burned, tongue sawn in half and arm graphically ripped off. Victim graphically mutilates their face with glass before having head gorily smashed. Victim is stabbed multiple times, has numerous nails driven into their body (including the eye), and has face beaten with a crowbar. Victim saws their own arm off, nails their face repeatedly and has their other arm gorily shot off. Humanoid monster gorily sawn in half vertically.
- Evil Dead Rise (2023) note For Ellie's repeated grisly injuries especially her Body of Bodies form in the climax. Otherwise a 9 for scalping, impalement, disembowlment, eye eating, throat tearing, child with arms torn off, etc.
- Exorcist: The Beginning (2004) note Gruesome and gory scenes, such as a mutilated corpse of a man hanging midair in the chapel and a stillborn baby eaten by maggots. Ditto for the Dominion: Prequel to the Exorcist version.
- Eyes Without a Face (1960) note Some of the most horrific and goriest Facial Horror imaginable, which includes an extremely detailed and graphic surgical face removal and a man being brutally mauled to death by a pack of large dogs that tear off his face in hideous fashion.
- Faces of Death (1978) and its sequels note In their entirety, the films consist of over-the-top scenes of graphic Gorn, with some of the footage being real (especially in the sequels).
- Father's Day (2011) note Onscreen rape male on male scenes, disembowelment, dismemberment, male victim's penis bitten off by the Big Bad, offscreen slaughter of female strippers, child's right eye cut in half with a knife, gory suicide pact by gun to go to hell (we see the half-blown up heads) and the Big Bad gorily shrunk until becoming a deformed newborn and subsequently repeatedly crushed to death.
- Feast (2005) note Repeated sloppy maiming. Including arguably the nastiest eye gouging ever filmed. All practical effects.
- Final Destination 5 (2011) note By far has the most brutal deaths in the series, and returns the darker tone. A cliffhanging victim is melted alive by hot oil before being de-gloved, another is nastily impaled on a sail mast (guts burst out in 3D), another is impaled by many pipes before smashing onto concrete, at least 2 are torn apart by falling debris (the pieces fly at screen in slow-mo), another is stuck by countless needles before having their head smashed. Other level 9 violence.
- The First Omen (2024) note Mainly here for a man getting impaled by a construction vehicle and the camera lingering on his entrails, showing he is Half the Man He Used to Be. Otherwise, level 9 for detailed birthing scenes, including one with a demon’s claw pushing out of the mother’s vagina (this initially led to an NC-17 rating), pictures of hideously deformed babies, a man getting the back of his head split open by a church roof with very brief sight of his brain, and a nun hanging and self-immolating herself. As a whole, considerably Bloodier and Gorier than other entries in the franchise.
- Flesh for the Beast (2003) note Extremely detailed gushing and disembowelment, guy vomiting his own intestines, priest dragged away whose arm is attached to another guy's arm, gypsy woman's sliced throat, black guy crucified on the ceiling and Gutted Like a Fish, naked succubi stained with blood while eating a human male torso (including his guts), the Big Bad is killed by being stabbed in the forehead with a scarab amulet.
- The Fly (1986) note Extremely detailed and graphic Body Horror, including a live animal hideously turned inside out and mainly the protagonist spending most of the back half of the film grossly, increasingly disfigured and then very gorily killed — and by then he has maimed two other characters, specifically dissolving the hand and ankle of one with corrosive vomit.
- The Fly II (1989) note Similar to above but maybe worse, as mutating protagonist brutally kills multiple characters — while still being presented as a hero — and two other characters (one an animal) are horribly mutated themselves.
- The Forest Of Love (2019) note Murdered bodies are disposed of in excrutiatingly detailed, sloppy, and drawn out fashion. Full dismemberment, gutting, blending, carving, crushing, and boiling organs and body parts.
- found. (2012) note A film-within-the-film depicts a slasher villain cutting off a bound woman's breast and sucking on the gushing wound for an extended period of time before repeatedly stabbing her to death, sawing her head off, very graphically gouging the eyes and eating them; the insides of the eyeballs drip out of his mouth as he chews amd finally has sex with the decapitated head onscreen. After that, the killer very graphically decapitates a tied-up woman before bathing himself in the blood squirting out of the neck and then having sex with the head while laughing maniacally. Despite being fictional in universe, the acts are too explicit and perverse for any level lower. Otherwise level 9 for a couple of decapitated, scalped and eyeless heads are seen, one of a young boy (borderline 10). Additionally, two mutilated bodies are briefly seen.
- Frankenstein's Army (2013) note Extended, gory brain mutilation to live, bound, conscious victim; extreme body horror. Other level 8-9 violence.
- From Hell (2001) note Hideously mutilated bodies. Most with throats cut, disemboweled and genitals mutilated; and one completely unrecognizable.
- From Beyond (1986) note Victim eaten to a hideously gory mess (all cuts), and still surviving. Eye grossly eaten out, and brain sucked through wound (uncut only).
- The Green Inferno (2013) note For Jonah's horrifically cruel and unusual death (while still conscious) before being cannibalized in extreme detail, and real-life stock photos of children being ritually genital mutilated. Otherwise level 9.
- The Gore Gore Girls (1972) note Faces mutilated beyond recognication, with the process being clearly shown.
- Grindhouse (2007) (Planet Terror) note People torn apart in extreme detail, disgusting body horror combined with very gory deaths, genital mutilation. Death Proof is on level 8 for a gory severed leg, and flattened face.
- The Green Mile (1999) note For Del's horrifically botched execution, wherein he's brutally electrocuted for several minutes at the electric chair while fully conscious and powerless, screaming bloody murder in utterly excruciating agony as the electricity destroys all of his nerve endings and begins incinerating his entire body from the inside out while he's still alive. The aftermath is similarly gruesome as well, showing an extremely disturbing close-up of his horrifically charred corpse, revealing that the burns have left his whole body completely unrecognizable. Otherwise level 6-7 for a couple electric chair executions that result in the victims having a few burn marks, Wild Bill's death, and the brutal (though thankfully off-screen) murder of two girls.
- Gutterballs (2008) note Excessively brutal, gory, and perverse killings include a face repeatedly grinded up, a penis slowly carved up in extreme close up, a mangled head with used condoms hanging out of it's eye sockets, a pornographic "69" with the participants forcibly gagged/suffocated to death on each other's genitalia by a third party, and probably worst of all, a horrific and nearly pornographic gang rape that lasts 10+ minutes and ends with the victim bloodily penetrated onscreen with a large bowling pin.
- Head (1968) note For real footage of Vietnam War violence, including the infamous "Saigon execution" which shows Nguyễn Văn Lém being executed by gunshot to the head. It is worth noting that the G rating in 1968 did not necessarily mean a movie was child friendly, but this might be the only G rated film to get here.
- Headless (2015) note Very gory mutilation, torture, and violation of victims in sequences even more drawn out and graphic than it's parent film.
- Halloween II (2009) note Naked woman's face smashed to a pulp. Incapacitated victim's head slowly carved off in close up. Innocent nurse stabbed innumerable times in the body and head. Man's eyes are gouged and he is then stabbed dozens of times. Head repeatedly stomped to mush.
- Halloween Kills (2021) note For the aftermath of a falling suicide; with the victim's guts, brains, and broken limbs on full display. Otherwise an 8/9.
- Halloween Ends (2022) note For the DJ who has his face smashed to a nasty pulp before having his tongue cut off and Michael's grisly Rasputinian Death at the end before being messily grinded up in a trash compactor. Otherwise 8-9 violence throughout.
- Hannibal (2001) note Extremely graphic sequence of Hannibal feeding Paul Krendler his own brain, with prolonged, detailed, live brain peeling, carving, cooking and eating being shown on full display. Mason Verger hideously mutilates his own face while drugged in a flashback. Pazzi, who's Bound and Gagged, has his stomach cut open and then thrown and hung off a balcony with a makeshift noose around his neck, causing all of his intestines to fall out and splatter on the ground below.
- Hatchet (2006) note Absurdly gory, brutal mutilations.
- The Hater (2020) note For graphic real-life footage of victims of terror attacks, including children. The mass shooting at the end is on level 8.
- Hellbound: Hellraiser II (1988) note Skinless bodies are graphically seen, alive and otherwise. Otherwise level 9.
- Hellraiser (1987) note Absurdly detailed and drawn out reconstruction of blood, to bone, to organ, to body. The resulting figure is a skinless body for a large chunk of the movie. Hideously melting live victims. Then said body has its flesh grotesquely mangled and is then gorily torn apart. Not to mention the gruesome mutilations of the Cenobites themselves and a few hideously brutal ones inflicted on innocents.
- Hellraiser (2022) note Very much in line with the original, except much worse, with more detailed mutilations and more gruesome kills than ever before. And that's not even mentioning the Cenobites themselves, who present body horror much more grotesque than that of their original counterparts.
- Hellraiser: Hellseeker (2002) note Brain surgery is performed on a conscious, restrained and unwilling man in graphic detail; a piece of the cranium is cut off exposing the brain which is pierced with a needle in close-up, causing the victim great pain (a soft 10, since this turns out to be a nightmare shortly afterward). Otherwise level 9.
- Hellraiser III: Hell on Earth (1992) note A restrained woman has her entire skin swiftly but graphically torn off by hooks (looks rather cartoonish, but still barely gets here). Otherwise level 9.
- Hellraiser: Revelations (2011) note A restrained man has his face slowly and graphically peeled off (a soft 10, since this turns out to be a nightmare shortly afterward). Otherwise level 9.
- Hellboy (2019) note Innocent victims gorily torn in two, skinned alive, skinned AND THEN sliced in two and much more (and that's not even counting the guys who deserved to get mutilated and so).
- Henry: Portrait of a Serial Killer (1986) note Brutal and sadistic torture/murder of innocent victims, including children. Although not overly bloody or gory, it still ranks here for the realism and pure cruelty of the violence depicted.
- The Hills Have Eyes 2 (2007) note Mutant has tongue bitten out, eye gouged, and head pulverized to mush. Another is gorily shot in the head, impaled, has the gory hole in their head mangled, their junk smashed and is then stabbed in the eye. Woman is savagely raped several times by 2 mutants. A very gory childbrith by a tortured woman, their face is then smashed. Gutted and mangled bodies and other nastiness in a cannibal kitchen.
- The Hills Run Red (2009) note Boy mutilates their own face with a scissor to a hideous mess. Woman bound gruesomely with hooks before being torn apart. Elaborate, torturous disembowelment. Skinned/gutted corpses. Other level 9 violence.
- Hobo with a Shotgun (2011) note Explicit genital mutilation, body repeated shot to pulp.
- Hostel (2005) note For a bound victim's eye torturously burned and then cut out; resulting in a pus geyser, as well as butchered remains of other victims lingered on. Other level 9 violence and more brief/obscured torture.
- Hostel Part II (2007) [R-rated and unrated version] note A chained up man is very graphically and bloodily castrated. The genitals are visible and shown close-up as they are torn off (this close-up is exclusive to the unrated version). A tied up man has a piece of his thigh very graphically sliced off and eaten by another man (this scene is identical in both versions, hence why both are level 10).
- Hostel Part III (2011) note For a torture scene in which a man's face is peeled off, with some very graphic close-ups of this being seen. Otherwise level 9.
- The House by the Cemetery (1981) note Woman graphically breaks her leg and is then torturously, gorily impaled repeatedly before being dragged away with even more mutilations. Gratuitously gruesome throat butchering. Other mutilated/rotting corpses.
- The House That Jack Built (2018) note Sadistic gruesome butchery, including children and subsequent abuse of the dead, animal cruelty, body dragged across pavement for extended period with the nasty results seen.
- The Human Centipede II (Full Sequence) (2011) note This time around, the creation of the centipede is done while the victims are conscious, and it is shown in excrutiating detail: Teeth are knocked out, ligaments are torn out and cut, buttocks are flayed and mouths are stabled to behinds. The people in the centipede are injected with laxatives leading to explosive diarrhea (this time, fecal matter is shown and highlighted). The person at the end of the centipede is also raped with barbed wire. Additionally, a woman gives birth and accidentally stomps the newborn to death (graphically onscreen). Although the movie is in black-and-white, it still reaches level 10 with ease. A colored version of the movie also exists.
- The Human Centipede III (Final Sequence) (2015) note A man is castrated with a knife: Mostly offscreen, but there are two brief but very graphic shots of the testicles being squeezed and yanked out. The barbed wire rape scene from the previous movie is briefly seen as a movie-within-a-movie. Otherwise level 9.

This includes a man in bed getting viciously stabbed several times in the face and chest, a head getting bashed repeatedly with a trophy in an insanely brutal fashion, resulting in lots of CGI blood and gore splattering everywhere despite the head itself not being seen, a head getting gorily and messily squashed with a single stomp, numerous gruesome compound fractures, an extremely graphically gory contorted body, a head shown twisted off in grotesque detail, Gabriel's explicit removal surgery and re-emergence, and horrific off-screen mutilations with plenty of blood, limbs and viscera seen everywhere.
- Man Bites Dog (1992) note For the woman gang-raped onscreen and disemboweled together with her boyfriend offscreen, the child suffocated with a pillow, the decaying half-buried corpses in a dried-up river and Benoit's girlfriend killed offscreen with her flute shoved up in her buttocks. The messy gunshots are on level 9.
- Maniac (Remake) (2012) note Three bloody and very graphic scalpings (in one instance, the victim is alive during the scalping, screaming in pain and terror). A prolonged scene in which a man is sadistically and very graphically mutilated.

- Parents (1989) note Gory cannibalism.
- The Passion of the Christ (2004) note Brutally gory, torturous overkill shown in lingering, excruciating detail. This includes Jesus being mercilessly whipped, beaten, humiliated, hideously mutilated and ultimately crucified throughout the film's two-hour runtime, being covered from head-to-toe in gnarly bruises, exposed bones, open flesh and other exceptionally graphic and disturbing wounds as a result while He's shown to apparently spill three times the amount of blood normally found in the human body. It overall goes as far as one can take level 10 due to the sheer inhumanity and unfathomable realism of the violence depicted (to the point that the Passion Recut version, which removes over six minutes of the most graphic violence and gore, still ranks at this level despite the most extreme moments being toned down), but the mere fact that all of this actually happened only takes it even further.

- Rambo (2008) note For the real life news footage showcasing atrocities committed in Myanmar/Burma as well as the massacre of one of the Karen villages and especially the final battle, with lots of hyper-gory carnage involving many enemy soldiers being graphically shot to pulp, mulched, liquefied, eviscerated, torn to shreds, ripped in half and overall obliterated into showers of gore by Rambo with a 50-cal Browning M2 machine gun. Tint's death also lands here, due to how drawn-out and brutal it is. Otherwise level 9.
- Re-Animator (1985) note Gory surgery on cadavers. Headless body's organs come to life and explode out of its body. Despite the Black Comedy, it's still brutally gory and detailed enough to reach this level.
- Bride of Re-Animator (1989) note Even nastier than the first movie.
- Beyond Re-Animator (2003) note Even Bloodier and Gorier than it's Bloodier and Gorier predecessor.
- Return to House on Haunted Hill (2007) note A man is suspended in mid-air by ghosts before being very graphically torn apart, resulting in an explosion of blood and gore; his gory remains splatter to the floor. A restrained man has his brain very graphically torn off while he is still alive.
- Revenge (2017) note Insanely bloody and brutal trauma with prolonged suffering and extreme emphasis on grevious gushing wounds, sometimes combined with rape and nudity.
- Riki-Oh: The Story of Ricky (1991) note Extremely creative, graphic, sometimes sadistic, ridiculous (but very serious) gore throughout.
- RoboCop (1987) (Director's Cut) note Murphy's longer death scene; involving being dismembered twice, shot to pulp and finished off with a graphic headshot. The theatrical cut is on level 9, with some mutilations being cut short or obscured.
- Sabotage (2014) note Body nailed to ceiling with intestines spilled and hanging out, as well as photos of similar scenes. Body mangled beyond recognition in train crash. Extremely bloody hard 8-9 violence throughout.
- The Sadness (2021) note Extreme, gruesome, gratuitous, sadistic gore, torture and perversion.
- Salò, or the 120 Days of Sodom (1975) note There's level 8 gory violence in the film, but it's fairly brief. What gets it up here is the constant use of rape in the most horrific of fashions
- Savage Dog (2017) note Man is incapacited with several bloody injuries and is then executed by shotgun to face (fully onscreen gore in unrated version). Man is brutally eviscerated after a fight and has his kidney pulled out and eaten in front of his eyes as he slowly dies (all versions). Many vicious and repetitive machete hackings/dismemberments, gory explosions, and shootings with tons of profuse squib work thoughout. Possibly the most gruesome Adkins/Johnson collaboration to date.

- Saw III (2006) note A man is bound by over a dozen chain hooks in his flesh and gruesomely tears them out one by one, only to be horribly mangled by a bomb. The aftermath of which is seen in extended and up close extreme detail. Bound victim has all limbs and head slowly twisted one by one until the bone sticks out.
- Saw IV (2007) note Extremely graphic, detailed autopsy gore. A restrained man is very graphically stabbed in the eye in slow-motion (blood is seen) before having all of his limbs torn out in quick succession; three of the dismemberments are very graphically shown as a lot of blood splatters on the walls.
- Saw V (2008) note A restrained man is torturously killed by being horizontally sliced in half by a swinging, slowly descending pendulum: Several of the swings are very graphically shown onscreen as the pendulum tears his abdomen open amid large blood splatters; some of the swings are shown very graphically tearing his intestines out.
- Saw 3D (2010) note The most brutal torture in the series hands down, all of it shown in visceral detail and resulting in all sorts of nasty deaths with plenty of gore abound. The most extreme instance is the "Horsepower Trap" scene, where a man is forced to peel the skin of his arms and back to reach the lever of a car only to run out of time, causing the jack holding the speeding car to fall which results in a victim having his arms and jaw ripped out, a female victim having her head and body grossly smashed by the car, another victim chained to the garage door getting savagely hit by the car which eventually crashes into another car, sending the man through the windshield to his grisly demise.
- Spiral (2021) note Victim has their tongue torn out before being gorily annihilated by a train, with the aftermath seen in extreme detail. Another body is seen completely skinned, with parts of the process shown onscreen.
- Saw X (2023) note For Valentina gruesomely sawing off her leg in extreme detail, being gorily decapitated, and then being dissected post-mortem to have her guts used as rope. Otherwise a 9.
- Scream (2022) note It ranks here for Richie's death, getting hideously stabbed numerous times all over by Sam in an extremely animalistic fashion with lots of blood spraying out, causing him to bleed out profusely before getting his throat gruesomely slashed open and finally being shot thrice post-mortem. Otherwise level 8-9.
- A Serbian Film (2010) note Sadistic, sexualized gory violence; including to very small children.
- Silent Hill (2006) note Mainly for when Pyramid Head kills a woman by skinning her alive (we even see the pelt being tossed and splatting on a church door) outside the church and Christabella's death by being raped and torn apart by sentient barbed wire.
- The Sinners of Hell (1960) note For the last scenes set in Hell, which includes various graphic depictions of flayings, eye-gougings, dismemberments, boilings, sawings, and worse. Otherwise, level 6.
- Slashers (2001) note Different footage of contestants being dismembered and disemboweled, The Preacherman stabbed into the right eye with a wooden dagger, Rebecca being sliced in half with a chainsaw, Rick disemboweled with a chainsaw by Chainsaw Charlie and both of them impaled with a bunch of spikes in a pit, Brenda getting beheaded by Dr. Ripper with a pair of big fucking surgical shears, Dr. Ripper stabbed with a pair of improvised wooden stakes by Devon and subsequently stabbed in the back by Michael, Michael committing suicide by self-beheading with the same weapon of Dr. Ripper before the end.
- Slither (2006) note Hideous body horror followed by very gory death.
- Smile (2022) note For the reveal of the monster's true form where it peels off all of its skin, along with a scene that shows the gruesome aftermath of a man's face after he was possessed to bludgeon himself to death. Otherwise a level 9 for a possessed woman slowly carving her face and throat open with a knife, along with other images and videos of prior "suicides"
- Snuff 102 (2007) note Graphic and possibly real footage of torture (fingers cut off in close-up, breasts nailed to a table) and mutilated bodies. Real and graphic footage of animal cruelty. Fictional violence is surprisingly tame overall for a film of this sort, but still gets here for a pregnant woman being raped with a knife (onscreen, but not that explicit) and a graphic eye-gouging. One of the hardest examples on the scale.
- Society (1989) note Copious gore, nudity, and incomprehensibly disgusting and psychosexual body horror (the most extreme instance being the shunting at the end, a gruesome blood orgy which probably has to be the most graphic and revolting orgy ever put to film, and the deaths of Dave Blanchard and Ted Ferguson via being slowly and painfully absorbed by the members of the Society, and dying via a hand up his crotch that grabs him through the mouth and eye sockets like a bowling ball before pulling him inside out, respectively, certainly help). While it is mostly played for Black Comedy, it's so grotesque that it earns this level.
- Southern Comfort (1981) note It gets rated this high for the graphic scene where a couple of actual pigs are slaughtered, skinned, and gutted. The violence against humans peaks at 8.
- StageFright -Aquarius- (1987, a.k.a. Deliria, Aquarius and Stage Fright) note Bloody stabbings and choppings, man drilled to death, extremly gory dismemberments with a chainsaw, man whose right arm is sliced with a chainsaw and then beheaded with an axe and a woman Gutted Like a Fish inside a shower.
- Starship Troopers (1997) note Extensively and hideously mutilated civilians after bug attacks. Action violence is on level 9.
- Street Trash (1987) note Insanely gory melting victims.
- The Suicide Squad (2021) note For the extremely gruesome displays of mutilated test subjects on a lab, with them in various states of grotesque decay including some with exposed viscera and entrails. Otherwise a hard 9 for many brief and often comical scenes of extreme gore, including a man vertically ripped in half in slow-mo, decapitations, Ludicrous Gibs, a body splattered across a window, melting, many dismemberments and more.
- Suspiria (2018) note Prolonged sequence where innocent victim is contorted in brutal and disgusting ways they survive and are painfully hooked in all limbs and then dragged away, ritual disembowelment, extreme head explosions.
- Taeter City (2012) note Bodies crushed, torn and mangled in the sloppiest, goriest ways imaginable.
- Tales of Halloween (2015) note Grisly torture of children. Disembowelment through the mouth, then body eviscerated. Etc.

- Terrifier (2017) note Gratuitously brutal, lengthy and outrageously bloody sequences of violence with grisly practical effects and no cutaways for the deaths whatsoever. This includes a bound naked woman getting sawed in half genitals first with organs continually spilling out as she wails, a girl getting repeatedly shot, a man getting impaled in the head and then having it sawn off with a machete while he gasps for life, the aftermath of a still alive woman who has been flayed off-screen, and live faces hideously mangled.
- Terrifier 2 (2022) note Very much in step with its predecessor, but goes even beyond that with multiple hideously graphic scenes of extreme violence and excessive gore. Stabbings and shootings with visceral detail are commonplace. A teen girl is sliced up, scalped, and skinned, with bleach and salt poured into her wounds. Heads are graphically severed and used as Halloween props. Corpses, both animal and human, are defiled. A man's eye is ripped straight from the socket and the killer pops it in his own eye socket. Another man is stabbed between the legs and has his genitalia severed. Live faces are hideously mangled on several occasions. A woman gives birth to a severed head. Although the grand majority of it is done in a much more exaggerated and darkly comedic fashion compared to its predecessor, it still reaches this level with ease.

- Texas Chainsaw Massacre (2022) note For the guy who has his leg twisted, his jugular stabbed, and his head bashed to paste completely onscreen; and the party bus massacre. Other level 8-9 violence.
- The Texas Chainsaw Massacre: The Beginning (2006) note Victim bound, their arm hideously mutilated, then torso gorily chainsawed, then face explicitly peeled off; etc.
- The Thing (1982) note Some of the most disgusting body horror imaginable. Specifically, the film resides here due to the extremely gory, drawn out and grotesque transformations of the titular creature. The worst scene may be when it consumes a kennel of dogs, leaving them partially liquefied and having to be put out of their misery.
- Thriller: A Cruel Picture (1973) note For the uncut version in which the rape scenes are accompanied by unsimulated and graphically seen penetration. Non-sexual violence is on level 8 for a brief but graphic eye stabbing.
- Till (2022) note For the aftermath of Emmett Till's brutal lynching, with several depictions of his horrifically mutilated and decaying corpse including an extremely gnarly head wound and especially his unrecognizably mangled face. So far, one of the only PG-13 rated films to rank here at this level.
- Tokyo Gore Police (2008) note Every brutal, gory mutilation known to man, alongside some of the most disgusting and disturbing body horror imaginable.
- The Toxic Avenger (1984) note Young boy brutally incapacitated, and his head then gorily crushed. Etc.
- Trauma 2018 (2018) note Abhorrent torture/rape/mutilation/murder.
- Trouble Everyday (2001) note For 2 sex scenes (one a brutal rape scene) that end in cannibalism; with the victims' faces, genitals, and bodies graphically gnawed upon as they howl in pain until the room is covered in their blood and viscera.
- Unplanned (2019) note Extremely detailed abortion related body horror; including real life images of aborted fetuses (some in late stages), prolonged depiction of the process, and other gore including lots of blood and viscera from genitals after surgical malpractice and counterceptive pills.
- The Untold Story (1993) note For the scene where a man kills most of a family in bloody and gory fashion, in which some of the victims include innocent, traumatized children. Otherwise a 9 for scenes where a man shoves a packet of chopsticks up a woman’s vagina (only the aftermath is seen, and not explicitly). - Wrong Turn 3: Left for Dead (2009) note A mutant kills a man by repeatedly stabbing him before very graphically cracking open his skull; his brain is exposed and the mutant eats some of it. A man's face is sliced off in gory fashion, and his corpse is subsequently mutilated by having both feet graphically sawn off. Both these scenes are instances of level 9 acts stacking.
- Wrong Turn 4: Bloody Beginnings (2011) note A man is quartered with barbed wire: His limbs are torn off one by one in very graphic close-ups, accompanied by huge amounts of dripping and splattering blood. A man is tied to a table with barbed wire and has pieces of his skin flayed off (very graphic close-ups) and eaten.
- Wrong Turn 6: Last Resort (2014) note A man has a fire hose shoved up his anus and is pumped full of water until his stomach very graphically and gorily explodes. A man is hung upside down and has his throat graphically and very bloodily slit before his stomach is very graphically carved open, causing intestines to fall out; cannibals gather under him and start drinking the blood and eating his insides.
- Zombi 2 (1979) note For the doctor's wife having their eye horribly mutilated in great detail and then being seen unrecognizably mutilated later. Otherwise level 9.
