Tropes:

  • Actually Pretty Funny: Megatron openly laughs at the prospect of Optimus dying from a virus after all the battles he's survived.
  • Adventures in Comaland: Megatron realizes he is trapped in a dream, and realizes that there's only one way out...
  • Arbitrary Skepticism: When going over how Bumblebee doesn't fit with the dream state, Megatron notes he doesn't believe in spirits.
  • Big "NO!": Megatron, as Bumblebee is pulled out of the dream state.
  • Blatant Lies: Just about anything Starscream tells Soundwave is this. Even though Soundwave's not stupid.
  • Death Glare: Soundwave's reaction to Starscream being content to let Megatron flatline. Starscream begrudgingly orders Knock Out to get to work just so that Soundwave will stop.
  • Didn't See That Coming: Megatron over an Autobot using a cortical psychic patch to get inside his head.
  • Double Meaning: The title refers to both Optimus being gravely sick from an old plague, and venturing into Megatron's twisted [sick] mind to get the cure.
  • The End... Or Is It?: Invoked as the Autobots and their human friends celebrate the victory. All of a sudden, Bumblebee momentarily freezes up, his eyes realign and the camera zooms into his head.
    Megatron: I'm out. Or should I say "in?"
  • The Fourth Wall Will Not Protect You: Look at this face - brr.
  • Internal Reveal: After sneaking aboard the Nemesis, Arcee and Bumblebee see that Megatron is comatose but still alive.
  • Irony: Megatron lampshades how Optimus survived countless years of fighting, only to be brought down by an ancient virus.
  • Journey to the Center of the Mind: Megatron's mind.
  • Kneel Before Zod: The dream state posits a scenario in which the Autobots were defeated completely and Megatron demands Optimus bow before him. "Optimus" refuses, so Megatron attacks him.
  • Large Ham: Megatron. Check out these gems:
    If this is my subconscious...WHAT ARE YOU DOING INSIDE MY HEAD?!
    No! TAKE ME WITH YOU! TAKE ME!!! [echoes "TAKE ME"]
  • Little "No": Ratchet after Optimus becomes infected.
  • Lotus-Eater Machine: Megatron dreams of perpetual fights with Optimus where he always wins. Bumblebee convinces him that it was ultimately empty.
  • MacGuffin: The cure to Cybonic Plague. And only Megatron has it, because naturally Megatron made the plague as bio warfare against the Autobots.
  • Mythology Gag: Megatron asks Bumblebee, "Do I still function?"
  • Nice Job Breaking It, Hero: Because Bumblebee is in Megatron's head and shaking things up, this is why Megatron is not unplugged by Starscream, and also how Megatron realises he's in a coma.
  • Nice Job Fixing It, Villain: Megatron shows Bumblebee the chemical formula to cure the plague infecting Optimus, not expecting him to know how to read it. His plan was to force Bumblebee to repair him, then he would give the cure to Optimus, but since Ratchet saw it through Bumblebee's eyes he was able to snap a picture of the formula and use it to cure him.
  • The Only One Allowed to Defeat You: Bumblebee exploits Megatron's desire to destroy Optimus to get the cure out of him.
  • The Plague: The Cybonic Plague, which passes as a result of contact with infected energon. It is purported to have killed millions during the Great War on Cybertron. According to Ratchet, it was created by Megatron, which is why Bumblebee uses the cortical psychic patch to go into his mind. Infected Cybertronians appear to slowly rust away in a horrific fashion while their Energon becomes tainted, with resulting discoloration resembling crude oil mixing with water.
  • Plot Hole: When Arcee unplugs Bumblebee from Megatron's mind, why does the whole city of Kaon disintegrate and the whole dream starts to crumble? Shouldn't only Bumblebee disappear from his mind?
    • It's implied Megatron was unknowingly starting to rouse from his coma, and in a panic demanded to be let out with Bumblebee.
  • Precision F-Strike: Well, not the F-word, but it's the first time in the show that Arcee uses a different swearword upon hearing Ratchet's plan.
    Arcee: Enter Megatron's brain? Ratchet, are you out of your fragging mind?
  • Shout-Out:
    • Cybonic Plague is an obvious take on Bubonic Plague.
    • Megatron while trapped in persistent vegetative state provokes a Terri Schiavo-esque argument between Soundwave and Starscream. Starscream wants to pull out the "feeding tube" Megatron is hooked on life to while Soundwave adamantly pursues preserving Megatron.
  • The Starscream: To finally become leader officially, Starscream cuts a deal with Knock Out. If Knock Out can convince Soundwave that Megatron's condition is irreversible and to consent to disconnecting him from life support, then he gets to become The Dragon to Lord Starscream. Knock Out wholeheartedly agrees.
  • This Cannot Be!: Ratchet's reaction to being told Megatron is still alive.
  • Title Drop:
    Arcee: Spiking hard... his sick mind is still at work.
  • Win-Win Ending: While the Autobots succeed in extracting the cure for the Cybonic Plague and curing Optimus in time, Megatron infiltrates Bumblebee's mind and actively begins work on restoring his crippled war-torn body.
  • Worthy Opponent: Megatron compliments Bumblebee for pointing out how killing Optimus in a dream will never have the same meaning to him as killing the real thing.
