Abby Fine gets a makeover using her mother's Kryptonite-based beauty treatment, but her new attractiveness turns out to have a price. Clark and Jonathan clash when Clark decides to join the football team again. Jason gets a job as Assistant Coach at Smallville High. Lois is forced to attend Smallville High to get the grades she needs to continue college.

- The villain, Elise Fine, is a cosmetic surgeon who has had work done herself, having undergone seven surgeries to get what she considered the perfect look. She performed an operation on her daughter to treat her acne using Kryptonite. Elise is shown as callous to her daughter's discomfort and the trauma other people have suffered because of her experiments.
- Her daughter Abigail herself is an inversion, being a kind person who is being bullied by others including her mother. It works, making her beautiful. It also causes her to drive anyone insane if she kisses them, due to the use of kryptonite during the surgery. She is portrayed as sympathetic throughout the episode.
- Rescue Reversal: When Clark frees Lois from Dr. Fine machine, it let's out kryptonite gas which weakens him enough for Dr. Fine to knock him out. But the freed Lois fights her off and drags him out of there.
- Shower of Love: Abigail has a makeout session with her former bully in the gym showers, during which she finds out the side-effects from her treatment. Amusingly, th shower part is accidental when she backs into the control valve as their kissing, but neither seems to mind at first.
- Strapped to an Operating Table: Lois wakes up in one after she's knocked out by Dr. Fine.
- Sweeps Week Lesbian Kiss: Abby kisses Lana (in order to infect her with the Kryptonite laced toxin).
- Teacher/Student Romance: What Lana and Jason's relationship becomes after he gets hired as an assistant coach at Smallville High.

- Villain of the Week: Dr. Elise Fine. She doesn't start out doing anything too heinous with her powers but quickly is willing to heurt people to cover up the side effects of what she does and vent out some old resentments.
- Undressing the Unconscious: After being gasses by Dr. Fine, Lois awakens to find she's been stripped down to her underwear and is strapped in Dr. Fine's plastic surgery machine. Clark arrives shortly after and frees her from it.
- What Happened to the Mouse?: Abigail disappears from the episode after she gives a toxic kiss on Lana and runs away crying. Chloe mentions she will back in school soon, but not if the side effects from her makeover will ever be reversed.
