- Broken Base: There is a noticeable split between how this game is perceived among fans of the original Twilight Syndrome series, and fans of Suda51. The former generally resent this game for numerous reasons, including the complete tonal shift in the writing in all but a few chapters, the change in the art direction, the lack of truly different outcomes despite the retention of Dialogue Trees, the inconsistent portrayal of the original game's main characters and their gradual loss of plot importance in favor of the new charactersnote , culminating in the decision to have them all Killed Off for Real by the end, leaving newcomer Ryo to confront the Big Bad— while the latter appreciate the game as an early example of Suda's quirky, surrealistic body of work. The way it tends to go is that the former are people who experienced Twilight Syndrome first (and thus are largely confined to Japan, though the advent of Fan Translation scripts/longplays on various sites means that the series is becoming more accessible for international viewers), and the latter are people who experienced Suda's later works first, facilitated by the fact that most of his games have been officially localized, starting with Killer7 in 2005.
- Hilarious in Hindsight: Mika's notebook containing her schedule in Chapter 9 mentions an upcoming "Clock Tower 3 release date", likely as a reference to the then-in-development Clock Tower II: The Struggle Within, a game that wouldn't be known as "Clock Tower 3" in any territories (that game wouldn't see the light of day until several years after Human Entertainment went bankrupt), which becomes retroactively humorous when one considers a certain series and developer which rose to prominence in the following years and whose apparent inability to release a third main numbered entry for their games has become a Running Gag among gamers.
