- Blue Baby, a corpse with cartoony X-eyes who can poop on command.
- Azazel, a winged demon who can spit blood lasers from his mouth.
- Tainted Azazel, who has had his wings and horn ripped off, but now has a very cute sneeze.
- The Lost, a wisp-like ghost.
- Tainted Lost, who has a cobweb on his head. A mod exists to give him a cat smile and ears.
- Lilith, another demon with gouged-out eyes and a little demonic familiar to fight for her.
- Tainted Lilith, who can now attack using her unborn demon child.
- Apollyon, a horned statue-creature with Empty Eyes.
- The Forgotten, a walking skeleton with his spirit still chained to his body.
- Tainted Forgotten, who is now permanently in ghost form and has a Badass Cape.
- Esau, who appears to be skinless and is depicted as being unwillingly dragged along by Jacob.
- Dark Esau, who is a spirit with flaming hair and a fanged smile.
Bosses
- Dingle, Dangle and Brownie, who are all happy living poops. Dingle and Dangle even give a Not-So-Innocent Whistle as part of their attacks.
- The Gurglings and Turdlings, walking piles of organs or poop with smiling faces.
- Baby Plum, a fat red fly with big eyes and a tooth.
- Little Horn, a small demon with a cute little fang and a habit of blowing himself up.
- The Haunt, who is a big Bedsheet Ghost... Until he takes off his mask.
- The Rainmaker, a stitched-up ghost with a flamboyant personality that makes him quite endearing.
- Min-Min, a tiny flame ghost who hides from you inside the Husk's corpse.
- Chub and CHAD, who are both happy, smiling, giant maggots.
- Gurdy and Gurdy Jr, both piles of organs with a little happy face on top.
- Big Horn, a giant demon who's fiercely protective of his little brother.
- War, who periodically falls over and makes Puppy-Dog Eyes at you.
- Hornfel, another small demon who taunts you throughout the fight and bursts into tears when defeated.
- Singe, a little humanoid creature who attacks with explosive farts.
- Loki, a four-armed demon with a constant Cheshire Cat Grin.
- The Siren, a Cute Monster Girl who has More Teeth than the Osmond Family.
- The Horny Boys, a stitched amalgamation of Loki and Hornfel with a massive amount of personality.
- Blastocyst, a giant cell with a face.
- Triachnid, a three-legged spider with a permanent Puppy-Dog Eye.
- Lokii, a bisected Loki who looks just as happy as the original.
- Krampus, a fluffy-looking fanged monster.
- Dogma's first form, which looks like a baby made out of static.
Items and Trinkets
- Dark Bum, a demon familiar that eats hearts... But only to turn them into a more useful type of heart.
- Lil Spewer, a worm-like creature that pukes on the floor, and makes an adorable noise when doing so.
- Incubus, another demon familiar with big, glowing red eyes.
- The Intruder, a happy-looking spider is oddly endearing, at least after it leaves Isaac to attack on its own. Before that point, however…
- Maw of the Void, which gives Isaac a happy smile with More Teeth than the Osmond Family and red eyes.
- Brimstone, which gives giant black eyes and horns.
- Lil Brimstone, which looks like the Brimstone costume but with a happy expression.
- Worm Friend, a tentacle that attacks enemies for you. Its icon is even surrounded by little hearts.
- Blood Puppy, a blood-covered worm with a cat smile that attacks you once it kills enough enemies.
