  • Approval of God: Doug Bradley (Pinhead's original actor) loved Pinhead's new design and Jamie Clayton's portrayal. Clive Barker was also very positive about her performance, even saying it inspired him to write more stories around Pinhead.
  • California Doubling: Belgrade, Serbia, doubles for Massachusetts.
  • Fake Nationality: Roland Voight, who is Serbian, is portrayed by Goran Višnjić, who is an American citizen born in Yugoslav Croatia.
  • Filming Location Cameo: The prologue is briefly set in Belgrade, Serbia, where the film was shot.
  • Older Than They Think: The idea of the pin-covered head Cenobite being female isn't as new as some viewers thought. Already in the original novella, The Hellbound Heart, the corresponding Cenobite was said to have a feminine voice, the only clue to a possible gender a human could perceive (making this movie Truer to the Text). But even more recently, in the Boom!Studios' Hellraiser comics, written by Clive Barker himself and released through the 2010s, Kirsty Cotton ended up turned into a female version of Pinhead when taking his place as leader of the Cenobites.
    • Made even more jarring that Kirsty's female version of Pinhead, in the Boom!Studios comics also wore a white robe to contrast with the male Pinhead's black attire, just like Cenobites in this movie.
  • Production Posse: Director David Bruckner reunites with writers Ben Collins and Luke Piotrowski. The trio previously collaborated on Siren and The Night House.
  • Queer Character, Queer Actor: Matt and Colin are played by gay actors Brandon Flynn and Adam Faison.
  • Saved from Development Hell: A Hellraiser reboot film had been in the works since Clive Barker announced production in 2006. French filmmakers Julien Maury and Alexandre Bustillo were originally attached to direct, but Dimension Films was dissatisfied with their script and delayed the release until 2009. After Maury and Bustillo left to work on Halloween II (2009), Dimension tapped another French filmmaker, Pascal Laugier, but Creative Differences led to him leaving the project as well. High Turnover Rate set in, with Christian E. Christiansen being announced as director in October 2010, only for Patrick Lussier to be announced as director mere days later. Barker then stepped into the directorial role in 2013 and announced that Doug Bradley would reprise his role as Pinhead, but didn't hear back from Dimension after submitting his script. It wasn't until the success of Halloween (2018) that the rights-holders took producing a Hellraiser reboot seriously.
  • What Could Have Been:
    • Before this film was conceptualized, several remakes went through the works with varying ideas in terms of plot and Pinhead appearance. One idea even had William Fichtner play Pinhead in a prequel movie.
    • Gottmik was at one point considered to play Pinhead.
    • Doug Bradley was almost brought in for a cameo but declined due to the COVID-19 Pandemic and his wish to leave his depiction of Pinhead and its legacy intact, which the crew accepted.
