The old lady turns out to be a powerful Humanoid Abomination with the ability to incinerate people alive. I then die just by looking her in the eyes.

Said scream came from a Banshee, a female spirit in Irish myth that has the ability to foretell the death of a family member. Right afterwards, a family member dies. Then the scream was heard again, and I was killed by a serial killer.
TNP is in a courtroom.

The courtroom is blown up after Phoenix Wright presents a bomb as evidence of my innocence for the trial which is about the murder of the previous poster. The bomb was armed and had only three seconds left until detonation when it was presented.
TNP is with ProtonJon, Chuggaaconroy, and NintendoCapriSun while playing Mario Party 3.

A serial killer dressed as a clown comes in and stabs me while the others escape.

The old children's story book, unfortunately for me, was something that Johnny the Homicidal Maniac was interested in and he kills me with several Noodle Implements.

I learn however that it's a Dead Man's Party, and in order to be let in I have to die. I suffer a fatal heart attack not long after.

TV Tropes ruins their life so bad they have to get a new one. But no one at the Celestial Bureaucracy is willing to give another life to someone who would just use it to browse TV Tropes all day.
