Pull from Ultimate Evil and NightmareFuel.Ruby Quest. The character page seems to be the best place for it. I can't view the NF replacement suggestion ATM, so I don't have an opinion on it.
As for Ultimate Evil, the trope is described as "evil so horrifying it cannot be shown on screen", so I'm going to propose we make it BUPKIS.
I'm good with leaving it on the character page.

While trope page images duplicating character images isn't a problem, I've never understood the current image, but I don't think this is BUPKIS-worthy. Dr. Claw from Inspector Gadget is a pretty well-known one (this is currently on TheFaceless.Western Animation, but I believe it's a better fit here):
Alternatively, maybe something of Fire Lord Ozai from Avatar: The Last Airbender, like [1]◊, [2]◊, or [3] (minus animation on the last one)?

"It's just a show; I should really just relax"I don't think either Dr. Claw or Ozai fit the trope description of "evil so horrifying it cannot be shown on screen". They're both simply regular humans who are depicted as The Faceless for dramatic reasons. Ozai's real face is shown at the end of season 2, and there's nothing horrifying about it. In fact, he's shown to be quite beautiful and non-evil looking, which would make it the opposite of this trope.

They're both listed as examples, though. Does this trope need cleanup? DRCEQ removed White Diamond from Steven Universe with the edit reason, "Not an example, especially how White Diamond was given more and more focus and screentime in the season finale," and her situation is very similar to Ozai. Dr. Claw's face was never shown in the series at all, only on an action figure and two video games.

Hmmm.. what about Gygas from the Mother franchise? In Earthbound, it's true form is incomprehensible to Humans.
Isn't it still possible for the trope to apply to her appearances before that? Tropes can list characters that used to apply. Giygas is more You Cannot Grasp the True Form, the trope it named. What about the Cloverfield monster or The Beast from Over the Garden Wall?
If Ozai and Claw aren't examples, does that mean Jaws isn't either, since it's just a giant shark?

That might work. The Beast actually is shown illuminated in a Freeze-Frame Bonus, and it is quite horrifying.

I am not a fan of 16. There's nothing ultimate about it—it's just some monster behind a dude. It's probably illustrating some other trope.

16 demonstrates that fairly well and gets a from me.
Migrated to Chloe Jessica!16 is illustrative, but we could use a collage of him from different scenes, so it would be clear that his lack of appearance is a recurring thing.

That exact same image can be found in, not one, not two, but three different articles. Specifically, this page, Ruby Quest (at the section for The Barbed Wheel) and Ruby Quest. Ruby Quest and this even have the same caption
For Ruby Quest, the page had this image linked in one example[1]◊. The last frame could work.
As for the other pages with that image, well I think it should probably stay on Ruby Quest. I'm not sure what Ultimate Evil's image would be though, got any suggestions?
