  • Dolled-Up Installment: The game was originally entitled "Virus", and did not star Mario. Wanting to recreate the success of Tetris, Nintendo changed the title and added a labcoat-clad Mario.
  • Keep Circulating the Tapes:
    • Dr. Mario 64 has only been released on the Nintendo 64 in America, on the iQue Player in China, as one of the games featured in Nintendo Puzzle Collection on the Nintendo GameCube in Japan, and that's it. It is one of the few N64 Mario games to have never been released on the Virtual Console. It would finally see a re-release as a launch title for the Nintendo Switch's collection of Nintendo 64 games.
    • With the Wii Shop Channel closing on January 30th, 2019, it means that the only way to play Dr. Mario Online Rx is to have a Wii where that game has been bought prior to that date.
    • Dr. Luigi and Miracle Cure have both become unavailable to download due to the closure of the Wii U and 3DS eShop.
    • Tetris & Dr. Mario has never been re-released likely due to legal issues with the Tetris half. Dr. Mario & Puzzle League has also never seen a re-release of any kind.
  • Meaningful Release Date: The Game Boy version was added to Nintendo Switch Online a few days after Mario Day.
  • No Export for You:
    • Dr. Mario 64 was not originally released in Europe or Australia. It didn’t see an official release in those regions until 20 years after its original debut. The original Nintendo 64 version was also never released in Japan, and has yet to see a re-release there.
    • Zig-zagged with Tetris & Dr. Mario. The full game was not released in Japan, but the Dr. Mario half was released on its own for the Satellaview and Nintendo Power services.
  • Port Overdosed: The Switch currently has three different versions of the game; NES, Game Boy, and 64.
