Twenty years after his first adventures, the Ranger once again finds himself at the entrance to a new world and new challenges.

This episode contains eight levels, plus a Secret Level: "The Military Base", "The Power Supply", "The Dark Days", "The Catacombs", "The Stronghold", "The Underworld", "The Otherworld", "The Year Zero".

This episode shows examples of:

  • Call-Forward: The first couple of levels take place in industrial areas that strongly resemble Quake II Strogg facilities, possibly implying a connection between Quake and the Strogg.
  • Chuck Cunningham Syndrome: None of the enemies and weapons from Scourge of Armagon and Dissolution of Eternity are present, only those from the base Quake game.
  • Cosmetic Award: The 2021 remaster offers the following achievements for this episode:
  • Hub Level: "Dimension of the Past" is the difficulty selection level.
  • Mooks, but no Bosses: Unlike the other expansion packs, this one doesn't end in any sort of boss fight. Instead, the final challenge is a gauntlet of Shamblers and Vores in an area that appears to be a revisited Shub-Niggurath's Lair, ending in a fight with a pair of Shamblers.
  • New Game Plus: In the original release, after finishing the episode, the player returns to the difficulty selection hub with all of the weaponry they have collected.
  • No Ending: In the original release, the episode takes the player to the difficulty selection area after completion. The 2021 remaster adds a congratulatory A Winner Is You text.
  • No Plot? No Problem!: Even less plot than the other episodes- since this one has only ever been released digitally, there is no manual or back cover text, and the ending “Congratulations” screen is just an out-of-universe message addressed to the player directly. The only thing resembling any kind of story is the familiar progression from sci-fi-themed idBase levels to fantasy-themed Medieval levels &, finally, to eldritch Runic levels.
  • Secret Level: "The House of Doom".
  • Sequel Escalation: The expansion pack takes advantage of the capacity of modern hardware, with more elaborate levels and much larger enemy groups.
  • A Winner Is You: In the 2021 remaster of Quake 1, finishing the episode leads to a "congratulations" text and the credits.
