- Acting for Two: Barnard Hughes plays both Mr. Drummond and Dr. Mallory, the surgeon who operates on the wrong patient (though with makeup, hairpiece and fake mustache he's not recognizable). He's only credited as Drummond, though.
- Dawson Casting: Diana Rigg, 33 at the time, as 25-year-old Barbara.
- Fake American: British Diana Rigg as Barbara, who's supposed to be from Boston.
- Mid-Development Genre Shift: Conceived as a TV series, but CBS rejected it. Paddy Chayefsky reworked it into a feature screenplay.
- Reality Subtext: Like his character, Scott was going through troubles with marriage and alcohol at the time.
- Underage Casting: Guernsey is in his mid-70s. He was played by 47-year-old Roberts Blossom.
- What Could Have Been:
- Burt Lancaster, Walter Matthau and Rod Steiger were in the running for Dr. Block.
- Jane Fonda and Candice Bergen were considered for Barbara.
- Write What You Know: Paddy Chayefsky wrote the screenplay after his wife was hospitalized and they grew frustrated with the poor service and lack of attention they received from the staff. Also, Chayefsky had a lengthy struggle with depression that informed Dr. Bock's own troubles.
