He basically influenced the US (and by extension, the world) psychiatric community's attitude to transgender people for most of the 20th century (and a fair bit of the 21st) with his theory of "autogynophilia" (which basically boils down to "trans women are gay men and trans women who aren't exclusively attracted to memen just have a weird fetish (because lesbians? No such thing!); there's no such thing as trans men or NB[s=]!".
Thanks to him, no trans person who didn't act like a 50's stereotype of their gender could get treatment, and were often forced to undergo conversion therapy until very recently.

It's been fun.Except the characters it is used for are NOT trans.
Ones like Astolfo are 100% self identifying as cis, they just cross dress.
Right, but the people using the slur don't recognize a difference. To them, transwomen are confused or malicious gay men.
The core meaning of the slur always was, and still is, "gay men who dress like women to trap real men". Why do you think there's such a strong push of the "it's not gay if it's a trap" mentality? Homophobia and disgust of transwomen have always been closely tied together.

It’s also intersectional with race, as Black Males are even less likely to seek help.

I think the point that's being made is that while they aren't trans characters in the source material, they're being treated as such in the "traps" meme, Immy.
Also, a quick PSA, since it happened a few times in the last few pages:
- The word transgender is an adjective. It's rude to use it as a noun (i.e. call someone "a transgender"); not unlike it being rude to talk about "the gays".
- Trans is an abbreviation of transgender and the same grammatical rules apply.
- It's "transgender [man/woman/person/etc]", with a space (e.g. trans woman, transgender man, trans people, etc). This is somewhat important since there are groups that try to say that transgender men and women "aren't really men/women" by treating it as one word.
- There's no such thing as "transgenderism" (the state of being trans isn't an illness or a philosophy, ta), and someone cannot be "transgendered" (as transgender is already an adjective, this is a hypercorrection).
- The antonym of "transgender" is "cisgender" (cis- being the opposite prefix of trans-).
- "Transsexual" is old fashioned, heavily medicalised and can be considered rude to use (not unlike calling a gay person a "homosexual").
