- Blooper: While being dragged out unconscious, Marsh's hand hits the frame of the door, and he clearly grabs hold of it. This might be explained as Marsh faking unconsciousness because he's actually Ocelot, but if the blow was faked, it wouldn't have made a noise.
- Box Office Bomb: Budget: $100 million. Gross: $51.1 million, more than a 75% drop from the previous film's $214 million gross.
- Sequel Gap: Came out nine years after The Expendables 3.
- Swan Song: It was the final time Alain Dorval dubbed Sylvester Stallone in European French. He had cancer and eventually passed away in February 2024.

- Brian Tyler was set to score, but due to his commitment to The Super Mario Bros. Movie and Fast X, he was replaced with Guillaume Roussel.
- The fourth film was originally envisioned as a spin-off starring Jason Statham as Lee Christmas and was given the Working Title A Christmas Story before eventually becoming the fourth main installment, but still with Christmas as the main character.
- Galgo, played by Antonio Banderas, was set to return in the movie.
- You Sound Familiar: Bernard Gabay is the main European French dub actor of both Antonio Banderas and Andy GarcĂa, and as such he voiced both Galgo in The Expendables 3 and Marsh in this film.
