As it turns out, the hospital has a website... where patients can make reviews of the doctors' work, and Bykov receives almost universally only complaints for him and his department. Kisegach orders him to improve the situation. He doesn't bother, and once they receive another complaint (for Lobanov), Kisegach gives him an ultimatum: either he finally starts working adequately, or she would make his life a living hell. But Bykov, in his typical manner, instead finds a programmer amongst his patients an asks him to hack the site. He utterly botches his job, resulting in seventy thousands completely identical positive reviews, which quickly leads to angry Kisegach summoning Bykov into her office.

Semyon's patient, the girl called Sasha, can't explain to her ex that she no longer wants to know him, and asks Semyon for help to keep him away; he suggests to instead get rid of him permanently — he can be very convincing. Unfortunately, Lobanov's attempt to scare away his patient's visitor backfires, as he accidentally attacks Sasha's father, who immediately complains to Kisegach, and she suspends Semyon from work. He gets replaced by Polina, who, after learning about the situation, offers her help — as a diplomat. But when she actually sees the guy in question, she thinks that he's perfect, and tries to convince Sasha to give him a second chance. Sasha isn't impressed, and requests another doctor. When Gleb comes to her, Sasha just tells him to not even try to speak with her ex, but he convinces her that he has a plan: he would pretend that he's her new boyfriend, and Sasha's ex would leave her alone.

Phil, after Gleb commented on his weird reaction to national anecdote, starts fearing that he may become too much Russian — and too little American. Phil decides to return to the USA; he already bought the tickets, and would not accept "no" as an answer. Bykov orders Rita, interns and other Therapy's stuff to put a show for Phil, imitating USA as much as they can, in hope that it would convince him to stay. It doesn't work, and angry Bykov just punishes him with a night shift and threatens to give him terrible characteristic if he tries to leave anyway; he also prohibits the other interns from adhering Phil's last party.

Kisegach notices consequences of Bykov's "USA hospital" experiment, and mistakes it as his attempt to finally improve, and even asks him to teach his colleagues how to do the same. When Bykov reveals what really happened, Kisegach hires that same programmer as their new system administrator... at the cost of Bykov's money bonus. Meanwhile, Phil's lonely party gets visited by... Sasha. This leads to them going wasted and falling asleep right in on-call room. This results in Phil missing his plane... and, accidentally, in Sasha being seen by her ex, who mistakes it for her sleeping with Phil and "dumping" her for "cheating", finally achieving her goal of getting rid of him. As for Phil, he missed his chance to leave, but no longer wants to: he realised that all his friends are here, while he has nobody to wait for him at home; plus, what he just did with Sasha, in the USA would result in him being fired, while Bykov would let it slide... for now.


This episode provides examples of:

  • Becoming the Mask: Phil starts fearing that he may become "too much Russian" and "too little American". Naturally, Bykov considers this to be a good thing. Polina gets surprised that he is American to begin with: she always thought that he was from the Baltic states. And when Bykov suggests a "fail-proof" method — choosing his favourite movie about the war — Phil names the Russian movie.
  • Cheaters Never Prosper: When ordered to improve the situation with the Therapy's quality of work under threat of being severely punished, Bykov decides to not bother with actually doing anything, and instead hires a programmer he found amongst the patient to hack it and falsify positive reviews. It results in him being busted (due to programmer botching his job), and said programmer being hired as system administrator at expense of Bykov's bonus money.
  • Digging Yourself Deeper:
    • When confronted about kicking his patient's father, Lobanov insists that he merely pushed him. Predictably, it only makes Kisegach angrier, resulting in Semyon being suspended from work.
    • Phil, panicking over becoming "too much Russian" and "too little American", decides to get drunk in order to think it over, which creeps him out once he realises that this is very Russian "tradition", and provokes him to buy the tickets as soon as possible; then, during discussion with Bykov about it, he realises that this desperate need to "go to America", which somehow would "solve all problems", is very much a Russian trait, too.
  • Dr. Jerk: Bykov's rude behaviour and general chaos in his department finally bites him in the ass; as it turns out, their hospital has a website... where everyone leaves reviews of their work, and Bykov receives almost universally only complaints. Kisegach orders him to do something to improve the situation, while Bykov states that they do not pay them enough to pretend that they like their clients in any way (and that sincere hate is still better than the fake smiles).
  • Epic Fail: A programmer whom Bykov hires to hack the hospital's website and insert several fake reviews, botches his job, and now their site has seventy thousands positive reviews... all of which are exactly the same. Kisegach immediately notices it, and calls Bykov out on it (with the said "programmer" arriving just in time to accidentally reveal his part in it).
  • Fake Relationship: Gleb pretends to be Sasha's new boyfriend in order to scare away the old one. It doesn't work, because the guy tests his knowledge of Sasha, and Gleb utterly fails, which convinces the guy that Gleb is lying.
  • Gone Horribly Wrong:
    • Gleb's plan how to deal with Sasha's ex utterly fails when Gelb gets forced to answer certain questions regarding Sasha (her birthday, her favourite flowers and where she studied), only to give the wrong answer on each one. The guy takes this as a sign that Sasha still loves him, removing the last doubts which he had left, and that this was just a plot to make him jealous. You know, exact opposite of what this plan was intended to achieve.
    • Bykov's "recreation of American hospital" fails to serve its purpose (to convince Phil to stay), but it impresses Kisegach... who now demands Bykov to help to organise other departments in the similar way.
  • Good Is Boring: The reason behind Sasha's ex-boyfriend actually becoming ex; while he seems to be pretty good and nice (which Polina instantly notices), Sasha considers him to be simply... boring, which she sees as sufficient motivation to stop their relationship.
  • Laser-Guided Karma: Instead of working on improving the situation in his department, Bykov hires a programmer to hack the website with reviews. This results in Kisegach busting them (programmer botched the task), and hiring that programmer as a new system administrator... with his salary being taken from Bykov's money bonus.
  • Mistaken for Cheating: As Sasha's ex still believes that he and Sasha are together, failing to recognise that she dumped him, when he sees her in bed with Phil (in truth, they just got wasted and fell asleep), he takes it as "cheating"... and dumps her. She plays along, to get rid of him for once.
  • Naughty Nurse Outfit: Rita dresses in rather revealing "uniform" when she acts as a part of Bykov's "recreate the USA for Phil" ploy.
  • No Accounting for Taste: Inverted, Polina questions just why Sasha dumped her ex, considering him being a perfect boyfriend. In truth, it's exactly the reason: she got bored with him being "too good".
  • Out of Focus: Episode is mainly centred around Phil, meaning that other interns have much less screen time. Even the plot about Sasha's ex-boyfriend rotates interns involved in it, and ultimately merges with Phil's plot.
  • Pet the Dog: Not wanting to just lose Phil like this, Bykov tasks his subordinates to "organise the USA" for him right here (by them speaking in English as much as they can, imitating American manner of work and even somehow obtaining the American crest), in hope that this would help him with his nostalgia and allow him to find the strength to stay (even Bykov acts much friendlier). It vastly improves Phil's mood, but ultimately fails at its one and only purpose — to actually force him to stay; and when carrot fails, Bykov tries the stick.
  • Poor Communication Kills:
    • Because Sasha never stated just how her ex looks like (and Semyon didn't bother to ask), Lobanov mistakes Sasha's father for him, and aggressively tells him to go screw himself, even pushes him. As result, the guy complains to Kisegach and Semyon gets temporarily suspended from work (barely avoiding being fired outright).
    • Because Sasha and Gleb never bothered to discuss their cover story about being in relationship, Gleb utterly fails with convincing Sasha's ex that he is Sasha's current (he doesn't even know her birthday), resulting in exactly opposite effect than what they planned to achieve: the ex interprets it as attempt to make him jealous.
  • Stealth Insult: Inverted; Bykov guises his genuine happiness with Phil choosing friends over his homeland under tons of insults and threats, but Phil, already knowing him good enough, instantly recognises it as a good sign.
  • There's No Place Like Home: Phil comes to conclusion that the only way for him to preserve his national identity is to return back to USA. Bykov convinces him to stay and give Russia another chance by recreating the similar environment right here, in their hospital, so Phil would accept it as his (second) home. It fails, forcing Bykov to start plan "B".
