As it turns out, the hospital has a website... where everyone leaves reviews on their work, and Bykov receives almost universally only complaints for him and his department. Kisegach orders him to improve the situation. He doesn't bother, and once they receive another complaint (for Lobanov), Kisegach gives him an ultimatum: either he finally starts working adequately, or she would make his life a living hell. But Bykov, in his typical manner, instead finds a programmer amongst his patients an asks him to hack the site. He utterly botches his job, resulting in seventy thousands completely identical positive reviews, which quickly leads to angry Kisegach summoning Bykov into her office.

Semyon's patient, the girl called Sasha, can't explain to her ex that she no longer wants to know him, and asks Semyon for help to keep him away; he suggests to instead get rid of him permanently -- he can be ''very'' convincing. Unfortunately, Lobanov's attempt to care away his patient's visitor backfires, as he accidentally attacks Sasha's ''father'', who immediately complains to Kisegach, and she suspends him from work. He gets replaced by Polina, who, after learning about the situation, offers her help -- as a diplomat. But when she actually sees the guy in question, she thinks that he's perfect, and tries to convince Sasha to give him a second chance. Sasha isn't impressed, and requests another doctor. When Gleb comes to her, Sasha just tells him to not even try to speak with her ex, but he convinces her that he has a plan: he would pretend that he's her new boyfriend, and Sasha's ex would leave her alone.

Phil, after Gleb commented on his weird reaction to national anecdote, starts fearing that he may become too much Russian -- and too little American. Phil decides to return to USA; he already bought the tickets, and would not accept "no" as an answer. Bykov orders Rita, interns and other Therapy's stuff to put a show for Phil, imitating USA as much as they can, in hope that it would convince him to stay. It doesn't work, and angry Bykov just punish him with a night shift and threaten to give him terrible characteristic if he tries to leave anyway; he also prohibits the other interns from adhering Phil's last party.

Kisegach notices consequences of Bykov's "USA hospital" experiment, and mistakes it as his attempt to finally improve, and even asks him to teach his colleagues how to do the same. When Bykov reveals what really happened, Kisegach hires that same programmer as their new system administrator... at the cost of Bykov's money bonus. Meanwhile, Phil's lonely party gets visited by... Sasha. This leads to them going wasted and falling asleep right in on-call room. This results in Phil missing his plane... and, accidentally, in Sasha being seen by her ex, who mistook it for her sleeping with Phil and "dumping" ''her'' for "cheating", finally achieving her goal of getting rid of him. As for Phil, he missed his chance to leave, but no longer wants to: all his friends are here, while he has nobody to wait him at home; plus, what he just did with Sasha, in USA would result in him being fired, while Bykov would let it slide... for now.

* LaserGuidedKarma: That guy Bykov hired to fake good reviews of his work on their hospital's website to hide Bykov's bad reputation amongst patients (which he botched and revealed whole plan to Kisegach) was ultimately hired by Kisegach as their new system administrator. And since whole situation only occur "thanks" to Bykov's antics, his salary would distracted from Bykov's money bonus.
* NaughtyNurseOutfit: Rita dressed in rather revealing "uniform" when she was part of Bykov's "[[ItMakesSenseInContext recreate USA for Phil]]" ploy.
* OutOfFocus: Episode is mainly centred around Phil, meaning other interns have much less screen time; besides Phil's plot, each one briefly appeared during Sasha's plot (which ultimately merged with Phil's one anyway), but otherwise none of them has their own story during this episode.
* PetTheDog: Not wanting to just lose Phil like this, Bykov tasked his subordinates to "organise USA" for him right here (by them speaking in English as much as they can, imitating American style of work and even somehow obtaining USA crest), in hope this would help him with his nostalgia and allow him to find strength to stay (even Bykov acted much friendlier). It ''vastly'' improved Phil's mood, but ultimately failed at its one and only purposed -- to actually force him to stay, forcing Bykov to find another way.

As it turns out, this hospital has web-site... where everyone leaves reviews on their work, and Bykov receives almost universally only complaints. Kisegach orders him to improve situation.

Semyon's patient, girl called Sasha, can't explain to her ex what she's no longer wants to know him, and asks Semyon for help to keep him away; he suggested to instead get rid of him permanently -- he can be ''very'' assuring.

Phil, after Gleb commented on his weird reaction to national anecdote, started fearing he may become too much Russian -- and too little American.

Unfortunately, Lobanov's attempt to scare his patient's visitor backfired, and backfired ''bad'', since it turns out to be her ''father'', who immediately complained to Kisegach, who instantly suspended him from work; he also complained on that same site brought up earlier. This was the last straw, and she gave Bykov an ultimatum: either he and his department finally starts working adequately, or she would make his life a living hell. Bykov, being Bykov, instead of actually changing himself, found a programmer amongst his patients and tasked him with hacking the site instead.

Since Lobanov is suspended from work, he is replaced by Polina, who, once learned about whole situation, immediately suggested her help; she considers herself to be much better diplomat.

Phil decided to quit working here, and return to USA. He already bought tickets, and would not accept "no" as an answer.

Polina finally met her patient's ex... and realised what he is next to ideal and tried to convince Sasha what she made a big mistake by dumping him. Naturally, Sasha was not impressed and demanded new doctor.

Bykov's "programmer" utterly botched his task (by increasing number of reviews up to ''seventy thousand'', with ''all of them'' being completely the same), resulting in Kisegach busting them both.

As was ordered by Bykov, Rita, interns and other Therapy's stuff made a show for Phil, imitating USA as much as they can, in hope it would convince him to stay.

After Polina, Sasha's new doctor is Gleb. She straight away ordered him to just not speak with her ex, ''at all'', but he assured her everything would be fine. He even has a plan already: the would pretend to be in relationship, so Sasha's ex would leave her alone.

Bykov's plan ultimately failed, because Phil decided to leave even after all what Bykov just done for him, which completely spoiled Bykov's mood to the point he punished Phil with night shift and threatened to give him terrible characteristic if he would ignore it. He also forbid other interns to visit Phil's last party.

Kisegach just noticed consequences of Bykov's "USA hospital" experiment, and mistook it as his attempt to finally improve, and even asked him to teach his colleagues how to do the same. When Bykov revealed what really happened, Kisegach hired that same programmer as their new system administrator... at the cost of Bykov's money bonus.

Meanwhile, Phil's lonely party was visited by... Sasha. This resulted in them being wasted and falling asleep right in on-call room. This resulted in Phil being too late on his plane... and, accidentally, in Sasha being seen by her ex, who mistook it for her sleeping with Phil and "dumping" ''her'', finally achieving her goal of getting rid of him.

Phil now missed his only opportunity to leave, but he no longer wants to, since all his friends are here, while he has nobody to wait him back home. He also realised, what back home what he just did with Sasha would likely result in him being fired, while Bykov would let it slide... for now.
