- The film opens with Father Costigan waking up in the middle of the night and discovering pictures of Katherine on fire, with only her face in the pictures burnt out, and the burnt pictures forming a satanic symbol.
- In Chile, the tunnels are packed with patients moaning in pain and suffering from hallucinations caused by contaminated drinking water. One man has some flesh burnt off right under his right eye from the toxic waste. The hallways echo with prayers and eerie chants for help, creating an almost claustrophobic feeling as Katherine and Ben move through the tunnels past the sick people.
- Ben explains that a month ago, the earthquake broke open the tomb of a priest that died 40 years earlier, revealing that his decaying body was preserved by the toxic waste spilling into Chile.
- What is making the villagers sick: every time they touch the dead priest's body and then place their hands on themselves, they're unknowingly infecting themselves with the contaminated toxic waste and getting themselves sick.
- Katherine and Ben exploring the dark cramped tunnels underneath Chile. A brief Jump Scare showing the floors just soaked in blood and dirt and toxic waste. Followed by screams echoing down the halls and roars and hissing noises up above.
- As Katherine investigates, she is grabbed and pulled through a hatch by two men in yellow hazmat suits who threaten her.
- Flashbacks to Katherine's time in Sudan. We see a Sudanese child's decaying corpse covered in flies, one of the casualties of the drought.
- How Katherine lost both her husband and daughter in a day back in Sudan: the villagers blamed Katherine and her family's arrival for the droughts so they killed her husband and daughter in a bloody sacrifice.
- Katherine sees her husband lying dead in the dirt and her daughter's corpse, preserved on a wooden table, blood dripping from her body down off the table and onto the floors. No wonder Katherine is haunted by that day.
- Per Doug's description, some of the men in the town of Haven went fishing in the rivers and found that the water had turned blood-red. That same day, a little girl found her brother dead on the river. The town suspects her of both his death AND turning the river red. And she's only 12.
- The scare Ben suffers when huge dead frogs crash into the river with noises like gunshots.
- Katherine takes a picture inside of a tree when Loren pops out screaming and grabs her. Katherine stumbles back and hits her head, making her experience visions of Loren standing in the river as it turns into blood.
- Doug prepares a meal for Katherine and Ben but as soon as he turns his back to check on the food, he finds it in a rotten state and swarming with flies and maggots.
- Doug, Ben and Katherine go to investigate a farm. Their truck is repeatedly rammed by a bull that is trying to kill them with his horns.
- The sight of many cattle dropping dead like flies one by one.
- Father Costigan has a frightening vision of an angel statue springing to life with its wings spread and turning into a swarm of flies.
- Doug's mansion experiences a blackout due to a thunderstorm. Suddenly Katherine's laptop shows a picture of Loren screaming with her eyes illuminated in red.
- When the thunder crashes and all the windows suddenly burst open due to the winds, startling Katherine.
- The idea of being stalked by a person who poses a threat to you while in a very vulnerable position. In Katherine's erotic dream, Loren is standing right in front of her watching. But Katherine is too preoccupied to do anything.
- After having a fever dream and nightmares, Katherine wakes up the following morning and starts making her breakfast. Only the egg she cracks open into the frying pan suddenly turns completely blood red. Then she's startled by the loud hissing sound of a boiling kettle of tea. Then she sees Loren standing right outside watching by the porch window.
- Katherine has a nightmare of being back in Sudan, cradling her husband and child's dead bodies. Then her daughter Sarah suddenly morphs into Loren, who tells Katherine, "He's right behind you". Then a man appears behind Katherine donning creepy face paint and wielding a butcher knife. Katherine promptly attacks the man for killing her family. He stabs her, blood drips from her mouth. Then she wakes up.
- The following morning, Katherine is relieved that the nightmares are over. Then she sees her footsteps in the kitchen are covered in white sand (from the nightmare of being back in Sudan) and realizes it wasn't a dream.
- Katherine tries to help Loren clean up the menstrual blood dripping down her leg. Then blood gushes into the towel she's holding against Loren and she has more frightening visions of people attempting to hunt Loren for killing her brother in the river.
- Loren's mother appearing suddenly, asking Katherine if she's going to kill her daughter Loren. Katherine assures Mrs. McConnell she won't harm her child and Mrs. McConnell asks Katherine "Why not?" Then they're attacked by a screaming Loren, who bites her mother's hand and pushes Katherine away.
- Brody's mummified rotted corpse with the eyes rolled back and his mouth wide open. The body's been dead for 3 days yet it's decomposed like 2 to 3 years instantly.
- Katherine reading the lab results and learning to her horror that the river is human blood. The whole river. And it's a two-mile stretch of river. And an estimated 300,000 bodies' worth of blood.
- As Ben, Doug and the sheriff locate Mayor Brooks to help evacuate the town, they enter the church to find the people dead inside, their bodies covered from head to toe with boils. Then they find a dying Mayor Brooks and his entire face is just covered with boils.Ben: "And there shall be boils breaking forth upon man and upon beast".
- As Father Costigan tries to warn Katherine, flames erupt from the pictures of Katherine, smoke and fire fill the room, and his door is locked. His dying screams for help go unheard and he burns to death.
- What Katherine finds at the McConnell cellar: a sacrificial chamber with an examination table, restraints for the arms and legs, two chalices, medical syringes and needles and a hole on the table for the blood to drip downward.
- For a moment it appears that Mrs. McConnell is going to murder Katherine, then she puts a gun in her mouth and shoots herself right in front of Katherine. Her blood pools over Loren's bare feet.
- Katherine is trapped in the basement and trying to escape without Loren noticing. Then her walky-talky goes off, alerting Loren in her direction.
- Loren stands outside her home, using locusts to attacking the townspeople who tried to shoot her. The swarms overwhelm the people, even killing a man in front of his son and Doug having to dive into the bloody river and Ben hiding in the mausoleum to free himself of the locusts, Katherine, who was trying to leave the McConnell home, is reduced to barricading herself inside Loren's bedroom with a mattress to shield herself from the locusts.
- What Ben finds when he goes off to investigate the mausoleum: satanic symbols written in blood, skulls of children with their birth dates embedded in writing on the skulls, hundreds of pre-pubescent children's corpses hanging up in chains and children's bones embedded in the walls of the mausoleum.
- As Katherine attempts to approach Loren in a friendly manner, birds and flies exit from the trees and into the skies. Great darkness falls over the area and fire starts raining down from the sky with thunder crashing.
- Katherine tries to kill Loren, and Loren shows Katherine through a vision what really happened: Loren is being held down by her mother and brother as they try to sacrifice her to Satan. Loren bites her brother Brody and takes off running. Brody catches up and stabs her with a butcher knife. Despite his young age he shows no remorse for his actions (he was obeying their mother's orders). The way Brody's body quickly decomposes is quite unnerving.
- After the reveal that Loren is actually Good All Along, Doug still tries to manipulate Katherine into killing Loren. It really shows just how lost and delusional Doug and the townspeople have become in their devotion to Satan.
- The reveal that Doug murdered Ben by bashing him in the back of the head with a vase just to keep him quiet about the satanic sacrificial chambers filled with children's bodies.
- After Doug is revealed to be the Big Bad, he just stands still and smiles eerily as Katherine calls him out. Even scarier, the blood he is covered in from diving into the river doesn't wash off of him.
- Katherine and Loren attempt to escape to safety, only to be surrounded by many of the townspeople of Haven. How do they escape? God shows up. First, He sends fireballs to destroy the townspeople. Then He takes care of Doug.
- Doug's death. His face decays rapidly due to the evil that he is, with his eyes turning bright red and the surrounding area turning black. Then the flesh melts off and he is reduced to a screaming skeleton until he combusts into ash and a giant fireball destroys the entire town of Haven.
- As Loren congratulates Katherine on her unborn baby, the ending shows flashbacks of what really happened.
- That erotic dream Katherine had? Not a dream and not erotic. In reality Doug raped a drugged and unconscious Katherine to create "a perfect child reborn with the eyes of Satan". And he remove all evidence of the rape before she woke up the next morning with no memory. Katherine then realizes to her horror that, since this is her second child, she is pregnant with the Antichrist.
