Killed the Fixer Elusive Target in Hitman (2016) and got a Silent Assassin rating for it. Apparently this one was the one with the lowest success rate or something?
Got 100% Completion in Shantae: Half-Genie Hero. I came close to doing it in Pirate's Curse, too, but I missed a few items along the way.

Level 31 in Elder Scrolls IV: Oblivion. I'm thinking of cranking up the difficulty level a bit.
Continuation of my prior post : completed Chapter 2, and more importantly GOT THE FREAKING Ribbon from the Bevelle Underground!

I recently got my smithing level up to 68 in Old-School Rune Scape. Now I can smith Mithril platebodies.
I had a stack of Mithril I was saving for this moment, and I made 50 sets of Mithril armor. I then sold those sets on the Grand Exchange for 339,000 gold in total. Those proceeds brought my in-game gold level to just over 3 million.
So that's two achievements - I can now smith any Mithril item I want, and I surpassed 3 million gold. I'm that much closer to getting an Old-School Bond.

Finally got gold on Midnight Run. But NOW I'm struggling with Dino Might! And I have the feeling Deep Trouble is going to be even worse. Fun!

Got Mehrune's Razor in Elder Scrolls IV: Oblivion. The number of vampires and human grunts I had to wade through to get it was ridiculous. But they're all dead or slaves so there!

Finished the main story in Monster Hunter World. There's still plenty to do, but it's still very satisfying.
Platinum trophy for A Hat in Time Trophy for Invisible Enemy mode in RE:Make. Trying to platinum, but got exploded during walking canister part for no save run and haven’t touched it since.

Now, this one isn't really a hard or particularly impressive accomplishment, but it was, however briefly, a fairly atmospheric experience. In Left 4 Dead 2, there's a level called the Sugar Mill; its gimmick, there are tons of Witches scattered around. I'm going to assume most of you are aware of Left 4 Dead and are probably are aware of what Witches are, especially given Steam kept giving the game out for free back in the day, but if you aren't: Witches are a special type of infected/zombie that look like starved, pale women with sharp, sharp bloody claws. They mind their own business, usually — some in this level sit still, some roam about — always with their heads in their hands, crying an awful, sombre and mournful wail. If you get too close, shine your flashlight whilst nearby or make a loud noise around them, however, they will jump at whoever alerted them and start shredding their new helpless victim to pieces in almost an instant.
So, like I said, a lot of Witches scattered all over the place, and I decide to go get the Sob Story achievement — "Navigate the sugar mill and reach the safe room without killing any Witches". I'm single-player (Normal difficulty), so I have three bots who are not going to let themselves die if a Witch comes, i.e. I can't alert any Witches whatsoever. I pass through most of the level alright, but then I reach near the end.
The final part of the Sugar Mill is to wander through a tall grass field — so tall, in fact, that you can see barely just a bit in front of you. You're surrounded by the cries of Witches, and those sobs are your only way of knowing where they are. I have a headset on, but I can hear them all over the place, and I can't pinpoint wherever they are. I wade through the field slowly, crouching, keeping an eye out just in case I stumble into any, and all throughout this regular zombies continue to roam. They're not much to deal with, but remember, gunshots can set off a Witch if you're too close. I continue on, cries still everywhere, 'til I make it to the end of the field and into safety. I never actually see a Witch at all. Achievement get, first try.
It wasn't a hard achievement, like I said, and if I hadn't just decided to do it on a whim, after having not played L4D2 for an age, and remembered exactly what it takes to trigger a Witch I probably wouldn't have had that much of a tense time. But between it all, and the constant knowledge of my teammates' unreliability, it was, indeed, tense. You had to be there.

Final Fantasy X-2 HD: Made it to Chapter 5, as far as I can tell, completely on track for 100% completion — in many ways, the worst ought to be behind me. Got an Episode Complete thus far for Besaid, Kilika, Luca, Mi'ihen, Mushroom Rock, and Bevelle. Currently attempting/planning to get the second Episode Complete for Mushroom Rock at the Den of Woe, although after today's marathon slog through Chapter 4 and everything above, I may need to give the game a rest for a while. (getting unceremoniously ganked at the last minute by That One Attack from Nooj isn't helping my attitude either)
