For starters, don't spoiler out the work's title like you did here. Second of all, the information you posted is vague enough that spoilering out the entire query was unnecessary. Finally, to answer your question, a warning at top would probably be best as you seem to be talking about a First-Episode Twist.
If it's relevant for the rest of the plot, we normally just make whole page spoilers-off and put a large "proceed at own risk" at the top.

Spoilers off should be a last resort.
Okay, let me describe it to you: Scott is killed in episode one, and the whole thing becomes an adaptation deviation and stealth sequel as Ramona investigates his death and we learn time travel shenanigans were involved.
The Owl House and Coyote Vs Acme are my Roman Empire.For works with a First-Episode Twist like School-Live! and Oshi no Ko, there is a tag at the start of the page warning that said twist will go without spoiler tags on the pages.

The anime Scott Pilgrim Takes Off has a massive twist in the very first episode that basically changes the entire trajectory of the story from its source material. How should we handle this on the work page so it doesn’t become a massive wall of spoiler tags? Put a warning at the top? Is there a forum thread we could consult?
