  • Blooper: The final two tracks of the soundtrack of the game is 23 minutes of outtakes from the game's voice actors, notably Jon St. John, Corey Cruise, Vince Desi, Rick Hunter, and Zack Ward.
  • Dummied Out: On the game's early access launch, the Shovel had a serrated head, but it was replaced with a Postal 2 style shovel by the "Tuesday" update.
  • In Memoriam: There's a number of graves in the cemetery in the Western town paying tribute to a number of deceased RWS alumni, plus Gary Coleman, Verne Troyer, Bill Kunkel, and Champ, as well as a few others, including Grant Imahara and Jeffrey Epstein.
  • Promoted Fanboy: Across the board:
    • YouTuber Civvie 11, prominently known for several Postal-related videos, appears as an in-game radio caller and as a NPC. Being Edensin's local crack dealer, because... what else would he be?
    • In a similar fashion, the Quality Assurance team for Postal 4 is comprised of longtime fans of the series, including two developers of Catharsis Reborn, a Source-based Postal fan-game.
    • Jake "The Voice" Parr and Gianni Matragrano, both fans of the Postal series, both appear in the game, with Jake voicing Tinklage and a radio DJ and Gianni voicing the Doomsday Cult leader and dam foreman on Friday.
    • "Kommander" Karl Schecht is also a huge fan of Postal 2, and in 2023, was officially hired by Running With Scissors as a 3D artist.
  • Refitted for Sequel: Several concepts, weapons and items that appeared or were scrapped from Postal III, such as the Rattler, the Mexican border town, a fast travel system, and the M60, were brought back and reworked for Postal 4.
  • Role Reprise: All three prior Postal Dude actors - Rick Hunter from the first two, Corey Cruise from III, and Zack Ward from the movie - returned to play the Dude as separate voice options after the game left Early Access.
  • Sequel Gap: Discounting the Paradise Lost expansion released in 2015, Postal 4 entered Early Access in 2019, making it the first major release in the franchise 8 years after Postal III in 2011.
  • Spoiled by the Merchandise: The soundtrack for the game has a song plainly titled "Dude Sr. Boss Fight Music", spoiling a major character formally introduced on Friday.
  • What Could Have Been:
    • Rick Hunter was in the running to reprise his role as The Postal Dude, with him having recorded some dialogue for the Monday errands, but scheduling conflicts with his radio career led to him dropping out.
    • After Hunter proved unable to return as the Dude, Running With Scissors then had serious consideration to bring back Corey Cruise for the role. According to Mike Jaret on a livestream, the reason Cruse didn't initially reprise the role for 4 was due to the negative experiences RWS had while working on Postal III, with the team deciding that a different direction should be taken with the Dude's voice entirely as a way of distancing themselves from III as much as possible. That said, they've stated that Cruise does have a role in the game, just not as the Postal Dude. Cruise would reprise the role in the spin-off, Postal: Brain Damaged, and his role in Postal 4 was eventually revealed to be as the Postal Dude's father.
      • Later totally averted as Rick and Corey were confirmed to be reprising their roles as voice options in the Leaving Early Access trailer. One could chalk this up to being a case of Lying Creator, though this was quickly addressed by Running with Scissors as they'd confirmed that Hunter really was unavailable for the previous two years of development.
    • The game was going to be funded via Kickstarter rather than being released on Steam Early Access, but Running With Scissors was on the verge of bankruptcy after the financial failure of Postal Redux, and the company needed money to keep the development going, which was why the game was released in a janky state for both the EA release and the 1.0 launch.
