Chimney ventures out to find Maddie. He ends up in Boston.
- The Bus Came Back: Eli Cobb is back, now working in Boston EMS, making his first appearance since "Chimney Begins".
- Call-Back: While Eli and Chimney give chest compressions to a man who has an axe in his head, Eli says it's like old times. Chimney's just glad nobody's puked on him yet.
- A Day in the Limelight: This episode focuses on Chimney and Maddie, after they've been absent for most of the second half of the season.
- Insistent Terminology: Athena checks up on Chimney via phone, wishing him luck in the State of Massachusetts. Chimney corrects her, as it is formally the Commonwealth of Massachusetts.Chimney: Apparently, they really care about that here.
- It's All My Fault: Maddie still blames herself for falling asleep and nearly letting Jee-yun drown in the bathtub.
- Pet the Dog: Maddie's landlord in Boston is clearly unsure when she asks to skip the paperwork for the apartment - until he realises she's in the therapy program, at which point he just nods and lets it go.
- Poorly Disguised Pilot: The episode serves as one for a potential Boston spinoff. During the 911 call the call soundwave is green, compared to LA's red or Austin's orange.
- St. Patrick's Day Episode: The episode takes place during the week of St. Patrick's Day, with all the festive emergencies that come with it, namely a bagpiper having respiratory issues due to mold, and an axe-throwing gone awry at a pub.
- Suck Out the Poison: Discussed. During a snakebite call, Chimney tells Randall that antivenom is a better option.
- Suicide by Sea: Maddie tries to do this in the opening by walking out into the ocean, an act brought on by her postpartum depression. After she's face deep and has several flashbacks, she changes her mind and checks into therapy.
- Title Drop: Several times. When talking with the therapist, Maddie says she's from Pennsylvania, moved to California, but trained here in Boston. Hen also does this when asking if Buck is sure of Maddie's location after talking to her. Buck mentions he heard the bells of the Old North Church.
- You Have GOT to Be Kidding Me!: Maddie has this reaction to finding out the clinic wants frequent urine tests.
