- In Your Body is Not an Apology, Santa Barbara PD has search warrants for Jude and Rachel under their dead names, Judith and Reuben.

- Played with in Retro Chill: it first appears that Evil Calvin calls Susie "Marcie", but he then reveals he was actually talking to Moe. He then calls him "Mack" and "Ginger".

- The Cutie Mark Crusaders constantly botch Destroyah's name, calling her "Destoroyana", "Desotroyah", etc. When they finally get her name right in chapter 23, she is very happy.
- Sweetie Belle calls Xenilla "Zenallia" in one scene.
- The ponies get confused by Ki Seong's name and call her "Tea Song" instead.
- In A Game Comes to Equestria, this is Hermos' problem with his first appearance in the story, having his name mislabeled as "Hymie" when he entered the rodeo. It isn't until the end of the whole event (and the events that follow) that everypony else gets to know his actual name.
- In Gensokyo's Heart, Cirno can't pronounce Patchouli's name right.Cirno: Not now, Path... Pac... Pactouli!
- Harbinger (Finmonster) (Danny Phantom, ParaNorman): Neil calls Dipper and Mabel "Dropper and Maple", which even Tucker, who doesn't know them at all, can tell is wrong even before Wendy corrects the boy.
- Harry Potter and the Natural 20: Milo is extremely prone to this, largely due to his unwillingness to pay attention to things he considers non-relevant fluff material. Victims include Gilderoy Lockhart, Dean and Seamus, and Dumbledore's lengthy list of titles. The one time he does get the last one correct, everyone immediately realize he's controlled by the Imperius Curse.
- In The Sponge House, a Fusion Fic where characters from The Loud House reenact Sponge Bob Squarepants episodes, the story "The Great Dog Race" (parodying "The Great Snail Race") has Mr. Grouse get misnamed as Mr. "Goose", "Grease", and "Gross".
- Turnabout Storm:
- Phoenix unintentionally misnames Fluttershy as Butshersty, Flutterscotch, and Firefly at 3 different times. The latter two just so happen to be names of ponies of previous generations of My Little Pony.note
- Lenora keeps referring to Twilight Sparkle as "Twilight Sparkler" for some reason. It gets on Twilight's nerves after a while, and she loses it after Rainbow gets acquitted.
- In Weight of the World, Canada has trouble remembering people's names, especially Pyrrha's. He calls her Pamela, Patricia, Penelope, and Pepper just to name a few. He eventually misnames her intentionally because it makes her smile. It's revealed Pyrrha likes it because his misnaming signified he and Alfred did not recognize her.
- A minor Running Gag in Danganronpa: Komm Susser Tod involves people, even childhood fans such as the protagonist Reiji, mistakenly referring to the Show Within a Show Shogun Yamato as Samurai Yamato, whether it'd be due to general ignorance or an inability to concentrate and remember.
- In the Facing the Future Series, the ghostly doberman of mechanic ghost Burner is called Desile. In life, Burner intended to name him Diesel, but dog tag he ordered ended up being misspelled.
- In Isabela's section of Sugar and Spice and Everything Nice
, it's a Running Gag that she can't remember the correct name of Josephine's betrothed, Lord Otranto. Throughout the narrative, she repeatedly (in her own head, never out loud) calls him something different each time, including Lord Oregano, Lord Oreo, and Lord Orko.
- In "Wayne Manor", part of the Sorrowful and Immaculate Hearts series, Martha Wayne initially appears to be a bubble-headed socialite who calls Alfred Pennyworth "Shillingsweight" and "Pennywork". An invoked instance, because she's a lot smarter than she chooses to appear, and once she decides to let him know it, she never gets his name wrong again.
- In the Meg's Family Series, Mayor Adam West always gets Maddie's name wrong, to the point where he calls her names that don't even sound remotely like her's.
- It's mentioned early on in The New Retcons that John's fellow dentist Everett had to take out a full page ad in the local paper to tell him his name was not "Elliot".
- In Ghostbusting 101, Venkman is drunk and fails to pronounce the names of Egon Spengler and Ryan Harness, calling them "Egosh" and "Shpengsh" and "Rine".
- In Here Comes the Boogieman, Danny calls Egon "Ethan" and "Eddie".
- In I Blame Myself, Oscar calls Winston "Inston" and Janine "Jenni". Justified, since he's only about eleven months old.
- The Hamsterball Show: Rachel Fink was accidentally called "Ranger Fink" by her younger sister Peepums once, and it stuck.
- In A Marauder's Plan, Sirius has a hard time remembering the name of Fudge's assistant after some inadvertent Dementor exposure. He successively thinks of the man as Marty, Monty, Murphy and Murray.
- In How Friendship Accidentally Saved Magical Britain, Umbridge accosts Tom at dinner, and he (genuinely) accidentally calls her "Professor Umbitch" with perfect sincerity, much to his classmates' horror and delighted awe. He decides to roll with it and insist that, yes, he was in fact referring to her, Professor Umbitch, which she responds to by attempting to curse him, prompting Moody to descend upon her and transform her into a toad. He doesn't even get detention or any kind of discipline for disrespecting a faculty member, and she stays the hell away from Tom for the rest of the school year. So it all works out in the end.
- In Weasley Girl, after Colin and Hermione have mentioned the Sherlock Holmes books to Ronnie, she misremembers the name and makes the occasional reference to "that Muggle girl, Shirley Holes." The first time she mentions the name is when she's talking to Dobby, and he seems to get the idea that this is what she'd like to be called. Hence, whenever he appears in the fic afterwards, he always addresses her as "Miss Shirley Holes."
- It happens once in a story of the Haunted Mansion and the Hatbox Ghost Fan Verse: when seeing the Ghost Host for the first time in "New at the Mansion", the ghost Purply Shroud mispronounces his name successively "Host Ghos" and "Spook Master", and then eventually gives up and decides to call him "Thing".
- For the Glory of Irk:
- It takes a while before Skoodge gets Dib's name right, instead calling him things like "Dab", "Dob", and "Dub".
- Xia is utterly unable to remember the name of her new underling Ven, calling him everything from the similar-sounding "Van" and "Sven" to stuff not even close, like "Tim".
- Gil the waiter refers to Therron (aka Tallest Purple) as "Tallest Periwinkle".
- Invader Zim: A Bad Thing Never Ends: Zim keeps referring to Aldrich Coathanger as "Coatrack".
- The New Adventures of Invader Zim:
- The actress that Viera interns for in Season 2 Episode 4 repeatedly gets her name wrong, cycling through every other name that starts with a "V". And then Rebecca, which an annoyed Viera notes doesn't even fit that pattern.
- In Season 2 Episode 11, it's mentioned that Purple has apparently been calling Bob "Pootchy".
- Top of the Line (Editor-Bug): The Tallest accidentally call Tak "Zack" during the final round of the SIR Unit tournament.
- In Itty Bitty Pretty One, Laverne's cousin Mary and Mary's daughter Angie call Shirley "Cheryl".
- The Awkward Adventures of Meghan Whimblesby include her not telling that she is from another world. So Meghan pretends to be from "Ravendill", her wrong name for Rivendell.
- In Groundhog GDIME, Charlize from England wants to visit the "Bruinduin", a river in Middle-earth. She is misnaming the Bruinen or the Anduin. Charlize seems to get her own wish and find herself in the Bruinduin, "a small, unimportant river in some obscure part of Middle Earth".
- Lothíriel by JunoMagic is about a woman from Germany. Many other Germans call her "Losíriel" because they can't say the English "th" sound.
- Not as Planned has a fan who inserts herself into Middle-earth. This girl misnames almost everything because she is too stupid to know the correct names. "Like, it's not my fault you can't use English names right." She says "Riverdale", "Bramir", and "Gamly", when the correct names are Rivendell, Boromir, and Gimli.
- In Anger Management, Carlitos calls Ronnie Anne "Wonnie Anne". Justified in that he's only learning to speak.
- The Boy Who Cried Idiot: Martin once calls Lincoln, "Abraham Lincoln".
- In The Devices and Machinations of Lisa Loud, baby Lily calls Leni "Eni".
- In How Many?, Stella is having a great deal of trouble getting the Loud sisters' names straight, and when going over them, she calls Leni "Lena", Lana "Lyla", and either Luna or Luan "Lulu".
- In Party of None, Leni calls Hank "Henry" and Hawk "Hubert".
- In Peeking Through the Fourth Wall, Lily has Elmuh Fudd Syndwome and calls Luna "Wuna", Luan "Wuan", Lincoln "Winky", Lana "Wana", Lisa "Wisa", and Rusty "Wusty".
- In Kedabory's Muppet Mania, guest star Kitsune Fox interprets Beaker's meeping as him giving his name as Mimi.
Magical Girl Lyrical Nanoha A's
- Similar to Vita in canon, Susanoo will never get Nanoha's name right in Infinity. He seems to have the general sound of it down, but tends to usually end up with something like 'Nanako', even when others directly point out to him he's got her name wrong.
- In Fragile and Adored, a doctor calls Peli Motto "Mrs. Djarin".
- A Scotsman in Egypt: Gordon of Edinburgh never gets his servant's name (Eoin) right. Of course, when Eoin accidentally calls him master George, he gets kicked out.
- Played for Drama in Occam's Razor, where Lila (or rather, Lie-La) was actually the charm-speaking Akumatized form of a shy girl named Vera. The fact that she had been Akumatized for months and spent that time as a Manipulative Bitch had left her horrified and shell-shocked. So when Kim absentmindedly calls her Lila, it causes Vera to have a panic attack and run off.
- Oversaturated World: Oversaturation: The human version of Twilight Sparkle needs a notebook to remember people's names after she meets them, calling Sunset, "Sunstorm" and Fluttershy, "Shutterfly".
- In What a Strange Little Colt, almost nobody pronounces “Gabriel” right the first time.
- A Dip in the Inkwell: The basis of the oneshot "Mr. and Mrs. O" is a new agent referring to Olive as "Mrs. O" instead of by her correct title, "Ms. O". The ending implies that this is a mistake that happens fairly often, enough for Olive to strongly suggest getting place cards for her and Otto.
- Meta example with Forever And A Mile: Opie is referred to as "Obi" on sheer accident, since the author originally thought that was how his name was pronounced by Oprah in "Ms. O Uh-Oh", his only appearance. All instances of "Obi" were later corrected to "Opie" after the author was corrected on his actual name (which is shown in the credits of the episode).
- A second meta example with OSMU: Fanfiction Friction: Oksana is repeatedly referred to as "Oskana", which is a result of the author getting her name incorrect.
- In Pokémon Reset Bloodlines, while Ash never calls Butch and Cassidy by name (as they never introduce themselves and he can't reveal that he knows them without bringing up his time-travel experience), he continually thinks of Butch by a different name throughout their first battle, thinking of him by terms such as 'Not James', 'Bart' or 'Dutch'.
- In The Trainer from a Far-Away Land, Satoshi can not pronounce N's name right, although it's due to the fact Kantonese has the same language limitations as Japanese, meaning he can't make the "N" sound. It happens so often that N gives up trying to correct Satoshi. Satoshi would pronounce N's name as Een-neh, Een-ne, Ennu or Enne.
- Don't Cry For Me, I've Already Wilted: In Chapter 8, Akaoni mistakenly refers to Saki as "Maki". Joker corrects him and snarks that she's not a sushi roll.
- Twinkling in the Dark: Majorina mistakenly calls Ichika Usami "Ichigo Masami".
- Ma Fille:
- Doubles as a Mythology Gag; in "Stage Name", Mr. Thompson refers to Piston Hondo as "Honda", which was his name prior to getting Screwed by the Lawyers.
- A darker example: in "Aunt Laura", despite the titular aunt wanting to file for sole custody of Katrina, she doesn't remember her name and calls her "Karina".
- In Widget & Clank: Lombax Bebop: Mr. Micron gets Widget's name wrong twice- first calling her "Wingding" and then, less than two minutes later, "Gidget." Widget calmly corrects him the first time, but gets fired up when he does it again, and loudly spells out her name letter by letter- only for him to whack her on the head with his cane for her trouble.
- In the prequel fanfic Prerugrats, both Tommy and the twins call Chuckie "Chubby" when they're first born.
- In Tommy Pickles: The Terrible Twos, when Tommy gets amnesia in "Tommy's Fear", he calls Chuckie "Chubby" and Lil "Lillip".
- The Ozpin Files: Ozpin forgets Professor Port's name and calls him Mr. Moustache for a while, and when he later finds out his name, he thinks it's 'Porter'.
- In Must Love Ned Flanders, Homer accidentally calls Naomi "Natalie".
- Similarly, in The Simpsons: Team L.A.S.H., Mr. Burns has an adoptive daughter named Anastasia, and Homer can never remember her name, instead referring to her with sound-alikes like "Angelica" or "Anesthesia". This is meant to mirror the original show's Running Gag of Mr. Burns not remembering who Homer Simpson is.
- In Empath: The Luckiest Smurf, Brave Scot Smurf Duncan McSmurf does not like being called Gutsy, a nickname that he got from Hefty for always having the guts to stand up to him. In "A Wild Winter Solstice", though, Papa Smurf accidentally refers to Duncan as Gutsy before realizing his mistake.
- Forum of Thrones: Initially, Marak mishears Noelle when she mentions the name of her god, R'hllor. In the following, he calls the god "rollmop", much to her annoyance. He eventually seems to catch up on the real pronunciation, but continues to misname it on purpose, to annoy her even further.
Star Trek: The Original Series
- In Rude Preschool Person, a Parody Fic lampooning the Fandom-Specific Plot where Kirk and Spock have sex due to the latter being on pon farr, the narrator gets several characters' names wrong due to being a toddler. She calls Spock "Pock", Rand "Barbie", and Uhura "Hurra".
- Discussed in the fan fic series TIE Fighter in relation to Chiss names and the difficulty humans have pronouncing the full versions and why they prefer humans just not try. “Pronounced improperly, [Thelea’s] name can inadvertently sound like our language’s word for a small tunneling rodent that lives in the high snow pack on our world.”
- In Claire the Kind, Claire has a habit of mispronouncing Jim's name before quickly correcting herself.
- Warriors: The Days The Clans Died: When asked who the previous medicine cat was, Sandfreckles guesses it was someone called "Jayclaw". She never met Jayfeather because he died before she was apprenticed.
- In Ripples, it's revealed that Miranda's real name is Mimira. "Miranda" is just what Cedric called her because he couldn't be bothered to remember her actual name; being young and impressionable, she decided that if he was calling her that, it must be a better name, and so started going by it.
- In Right Minded, Taylor's brain damage makes her incapable of correctly or consistently speaking proper names: she will call any person, place, or organization by a different wrong name every time. On top of that, she hears the names she meant to say, meaning that she has to rely on her teammates to tell her what's actually coming out of her mouth.
