Even if it's an incredibly direct translation (i.e. the aforementioned Pharmacy in Another World)?

Nah, we only go by official translations; if there isn't an official translation (or if the official translation uses the foreign title) then we stick to the foreign language title, even if it's a very simple one like this.

Even then. See Baccano as an example; it has a relatively direct translation in the form of "ruckus or racket", but instead the Italian word is preserved. YuYu Hakusho easily translates to "Wooo! Wooo! White Paper". Again, we don't know how it might be translated and we're not in the business of making up names.

I believe direct/fan translations can at least be used as redirects, but not as the name of the page proper.
