It's confusing and I don't even see flirty there.
My attention span isn't long enough to read through all that. A good personal rule of thumb is that if I have to read the picture, I might as well just read the trope page.
I'm not sure if including the "stepsiblings" part in the page image is necessary. Since it says "Flirty Stepsiblings" right at the top of the page.
So, if the problem with the current image is that it doesn't illustrate the trope well enough, that argument seems a little suspect. If the problem with the image is just that it doesn't add anything to the page, I guess I can't argue, and there may as well be no image.

The Mysterious Stranger: Satan.Even if the pic isn't ideal, it shows they're flirty, and judging from the name of the trope, it's not to hard to figure they're step siblings, so yes, I would say Current > nothing > suggestions so far.
I don't get flirty from that, at all. Also ... is that one of those "read it from the right" things? Gotta remember that only hardcore manga fans are going to know that.

Ever since they started releasing manga in English non-flipped, the back, or should I say "front" pages have been dedicated to pointing out the correct reading direction.

I still agree that it's not a good image. Though, unlike most people here, I'd opt for imageless, given how nothing's actually going to say "stepsiblings".

Just two random people. Nothing about them seems to suggest that they're stepsiblings. It even sort of looks one-sided from the girl's perspective.
I can't think of a convincing way to display this trope, so it doesn't really need one.
