  • Complete Monster: Dr. Horn's Evil Twin is the youngest—and most sadistic—member of the Royal Medical Society, having participated in the conspiracy that drove Patrick Heyworth to kill hundreds of people. Using his good-hearted brother to get clinical records about pregnant prostitutes, he performs horrific surgeries on them to extract their babies for Lord Leslie's experiments, even trying to kill not only Koudelka but also his own brother for discovering the truth. While having the pursuit of science as his primary reason to be running the operation, he also admits to loathing prostitutes, and uses his position to satisfy his own hatred against them.
  • Enjoy the Story, Skip the Game: The game's story and the interactions between the three party members is often considered a highlight. It's just a shame it's attached to a very poorly designed JRPG battle system, and thus many Shadow Hearts fans suggest you watch it in a Let's Play format.

  • That One Boss: Alias, a gunman who ambushes you on a staircase. He runs back and forth between two boxes, shooting at you and usually dealing enough damage to kill whoever he hits. But you can't reach him with a melee attack until you destroy the boxes, and he's somehow immune to magic, meaning he'll continually pick you off as you work on destroying the boxes...
  • Wake-Up Call Boss: The Guardian Priest who waits at the end of the underground tunnels. It can take a lot of punishment and dish it out with both its physical and magical attacks, including a combo physical attack that can deal over 700 HP in damage at a point in the game when that much damage is likely to be instantly fatal to at least one of your party members even at full health. Also he often inflicts poison and silence with his attacks. He's likely to be the first fight that cannot just be dealt with through brute force or exploiting elemental weaknesses and that will make the player manage status ailments, use buffing magic, and carefully balance their offense and defense.
