** And then there's of course the big Player Punch regarding Seiko herself. It starts turns out bad, goes to worse that Seiko's death was not a suicide, as initially believed, but a case of Naomi becoming possessed by the Darkening and putting a noose around Seiko's neck. Not only does Naomi realize that she is at fault for her best friend's death, if the player doesn't manage to do the right thing and get the perfect best ending, it will result in Naomi dying, as well.will attempt to commit suicide by hanging herself.
** The first chapter of Book of Shadows revolving around Naomi and Seiko, with the former realizing that they've been in this situation before and that her fight with Seiko resulted in her friend's death. Trying to avoid the fight doesn't work, and so Naomi has to end up finding Seiko before she dies. The true ending of the chapter is a cruel HopeSpot because Naomi succeeds in rescuing Seiko from her death, only for Seiko to freak out when seeing Naomi and running away, before falling down a flight of stairs and losing her head.
*** Immediately after seeing Seiko die again, Sachiko appears and mocks Naomi -- and the player -- for thinking they could avoid the terrors of Heavenly Host. Sachiko reveals that any person that died in the school is fated to repeat their deaths, no matter how many times you might try to change things. And those deaths tend to be worse than the original one.

** Phoenix Wright: Ace Attorney:
*** Mia's death near the beginning of the game is a bit different from this trope's norm in the sense that you don't really get a sense of how awesome she was until after she dies- though it still hits hard as she's the player's co-counsel in the tutorial and you first encounter her little sister crying over her dead body. In any case, Redd White is quite likely the most hated villain in the series who isn't a final case villain.
