Ha, already have one. Dr. Esther Carlotta, a geneticist who bio-engineers Child Soldiers for the New Blood program. One of them, the 11 year old Angel Baby, is her foster son/sex toy. She's dangerously obsessed with her "beautiful little baby."

I already have one as well, a Big Bad for my now-on-hold 4th Edition D&D game: Nailan, the Mask of Pain, who makes a living off of taking requests to hunt down targets and bringing them back to his headquarters. Anyone who is captured by him can be tortured by anyone who pays. Lethal torture and singular rights to a victim cost extra. He also actively hunts down powerful figures who would protect the weak, putting them in the torture chamber as well.

I don't want to think of another right now because it might feel forced.

Well, the most disturbing villain would have to be tailored to the reader. For instance, I find rape to be the worst crime possible, if committed...I find the only word that works here is "properly". So mine would, in theory, have to be a serial rapist of Anything That Moves for the hell of it, but the problem is that a realistic villain will always be more disturbing, so that doesn't quite work.

So what I would do is start with a perfectly normal person, and meticulously catalogue how, each day, the character gets a little worse. No one change is unreasonable, but by the end of the story, we've seen this person go from someone you could pass on the street to a paranoid freak who kills/maims/rapes everything, not because of any particular malice, but just because the, ah, 'villain' is confused and scared.

Or, if I tailor the story so that the reader matches myself: You.

I just thought of these guys.

Although I once had a very disturbing villain in the form of an Evil Princess who used Mind Control and trickery to make people think she was an innocent Princess Classic, and who was responsible for, among other things, having an honest knight who tried to resist her tortured and turned into a eunuch, then keeping him in fear with Mind Control.

...I'd say that's pretty disturbing.

Hmm... Let's see...

Alright, take this scenario:

Two twin brothers. They were both totally normal teenagers, living with their single mother... And then a celestial figure comes down, killing their mother but granting both of them fantastical powers.

One twin blames the other for their mother's death. The other is just a Crouching Moron, Hidden Badass that wants to help people no matter the cost.

Well, then we eventually see the first twin spiral out of control into an incurable madness which leaves him seeing his brother in the faces of everyone he passes by. He doesn't know why, it's just like he see's a part of him in everyone...

That's when it hits him. He'll never be happy until he tracks down his real brother. His solution? Kill everyone he sees until he finds the real one.

The most disturbing villain I can think of would be Armadillio Regold, who has The power of god which makes him immortal, unfortunately, to keep his people in the dark, he still has children and "heirs" and ends up having to body steal one and he kills all the rest in secret. (Blood kin are the only ones allowed access to the power thingy.

A thief who tried to steal the power of a god, and succeeded. For one hellish moment, he saw and knew far more than his mind could withstand, but now he only sees the products of his twisted imagination. He wanders at random, never speaking but often laughing, and he's equally likely to give you a flawless diamond or turn you into a life-sized statue made of licorice. Many people have tried to kill him, but he doesn't seem to have noticed.

Then throw them into a blender, make the mass of flesh human and human-sized again with no true redeeming qualities AT ALL (he/she/it can fake them), and that's the most disturbing villain I can think of.

I think of an entity that was some kind of bio-engineering project Gone Horribly Right. It was made as a super soldier who could mend any injury by grafting tissue from slain foes onto its own body, becoming virtually indestructible. However, this creature gets the idea that its body is 'wrong,' or 'imperfect,' and soon escapes containment to wander the streets, searching for more 'perfect' people to fuse with. It eventually becomes a huge, misshapen blob of flesh, and spends all its time ripping parts off and grafting new things on in hopes of fixing itself and making its body perfect.

,,,That sounded a lot more interesting and original in my head.

A hole in the ground. There's an infinitely long tentacle in this hole, and one of the ends is sticking out of the hole. On the end of the tentacle is a human hand, and on the palm of the human hand is a mouth full of razor sharp teeth. It can eat any organic matter and as it does so, the diameter of the hole slowly increases. As the diameter of the hole increases, more tentacles like the first one can come out.

I'm feeling strangely happy now, contented and serene. Oh don't you see, finally I'll be, somewhere that's green...

I have one in a Sailor Moon fanfic. It's basically Sugar Apocalypse taken literally. The Big Bad wants to be able to turn people into candy/dessert materials like cookies in order to eat them. And you can still feel pain and are conscious if you're turned into candy, so she wants to eat people alive. And then do this to the ENTIRE WORLD (yes, the Earth itself). Luckily at the beginning of the story she is only able to turn non-living objects into candy and she can't do things that are too big yet, but if she were to steal the Silver Crystal she would gain the ability to turn living beings into candy and eat them alive (and use her powers on much bigger objects such as buildings and pieces of the ground).

Basically combining I'm a Humanitarian, Sweet Tooth, And I Must Scream, Level Ate and a literal version of Sugar Apocalypse (turning the world into candy in a way that would destroy all life). I don't know if she's the MOST disturbing villain I've created or can think of, but she's pretty high up there, at least.

My Mad Scientist who creates Ridiculously Human Robots out of students, while his brother is The Casanova and created a clone of his girlfriend, inducing a Heroic BSoD in her and raising the Clone as his own sister *shudder*.

My other villian is a Doctor who takes time by slicing people while they're sleeping and removing their organs
