There were a lot of crazy things changed by this. My theory is that thanks to the two of them repeatedly setting off Thawne's radar after going to the past, Eobard pulled a Kill and Replace on Sherloque in the current timeline sometime after they called him in to face Cicada so he could observe current events.
Either that or he was Sherloque the whole time (or more accurately, that even before they messed with the timeline again he was masquerading as Sherloque), which honestly I like a little better even if it is a bit less likely.

The scene of Barry and Nora hugging was really emotional and then it got better with the particle accelerator exploding and "everything beginning" — and we see everything, Barry getting hit by lightning, Mark Mardon's plane going down, Professor Stein with the F.I.R.E.S.T.O.R.M. Matrix, DeVoe with his thinking cap, and everything else.
Unfortunately, I think this is also where Cicada's story should have ended. There really isn't anywhere else it could go — after all, we already know his origin, he doesn't really have any plans other than "kill all metahumans," so where else are they going to take him? And we already had the perfect way to stop him too: Killer Frost. But he...forced jumped away? I guess?
I like that Thawne is helping Nora from the future. It really introduces a whole "Hannibal Lecter" vibe and was already hinted at before with Sherloque. Who is also probably Thawne. So Thawne manipulated Nora into going back and meeting himself...twice.
Remember, Nora was the one who suggested bringing Sherloque to the team (or at least recruiting an alternate universe Wells), so if he is Thawne, it's likely she deliberately orchestrated him joining the team (though without his present-self's knowledge, apparently).

Whatever version of Thawne/Wells is in the 2049 prison cell has a lot of gray hair, so we can assume he's a lot further ahead in his personal timeline than whatever Thawne/Wells we're dealing with now.

I know things have been wonky with him ever since Flashpoint, but I thought that was finally done with after Legends Season 2...though that did stop him from showing up during the Crisis on Earth X crossover.

The Thawne in Crisis must come after Flash Season 1, since he does have Wells face. But that Thawne was erased from the timeline. And yet, he can't be erased, because Barry's mother still died, the particle accelerator still exploded, etc.
Basically, Thawne is a living paradox now.
I think the implication is that when the Black Flash caught up with Thawne, it 'killed' him and imprisoned him within the Speed Force.
Then, when Barry escaped the Speed Force at the beginning of season 4, Thawne was able to as well.
Since Thawne's a time traveler, him showing up in "Crisis on Earth-X" only means he gets brought back to life at some point. If you've watched the full trailer for "Elseworlds" the Reverse Flash with Wells's face appears in it, so maybe all the reality warping shenanigans bring him back to life, and he then travels back in time a year to team up with parallel universe Nazis.

This is the perfect time for a Hope Spot where the season's villain is conclusively defeated, only for Thawne to be revealed. But this show has always had a single overarching villain per season, so I have a bad feeling that they kinda... forgot? That they can just finish off one villain early and then move onto another?
Maybe they'll have some awesome stuff for Cicada later, but I don't have high hopes. Maybe Grace wakes up from her coma as a metahuman and Cicada has to deal with his entire morality being upended. And then comes the reveal that the dagger was controlling him or whatever. I dunno.
You know, the more I think about it, the more I realize that Buffy the Vampire Slayer really did have the best way to handle villains — you start the season with the "Little Bad" and then when they are defeated, you transition to the "Big Bad."
Well they kinda, sorta did that with Savitar.
