A prison inmate is murdered and upon visiting the murder scene, Jane and Maura discover that Hoyt has also taken up residence in the prison's infirmary, supposedly suffering from late-stage cancer. Hoyt offers up information to Jane on more of his past murders, but his vague clues lead Maura to believe that the cancer has rotted his brain. Meanwhile, Maura is having trouble deciding on a birthday present while she and Angela try to plan a surprise party.

- Asshole Victim: The grand total of people who would mourn the death of serial killer and rapist Charles Hoyt is about...zero.
- Dirty Cop: The killer and Charles' new apprentice is one of the prison guards.
- Pre-Mortem One-Liner: "I win! [stabs Charles in the chest] And you're going to Hell alone."
- Sword over Head: Jane has Charles pinned to the ground while she's holding a scalpel. Unlike most examples, Jane follows through.
- Terminally-Ill Criminal: When Hoyt finds out he's dying of cancer, he attempts to kill Jane and Maura to go with him. Thankfully, they figure it out, with Jane finishing the bastard off.
- Victoria's Secret Compartment: Maura smuggles a vial of Hoyt's blood out of the prison in her bra. Nobody expects anyone to smuggle objects out of a prison most of the time.
