A groom (Ted) collapses at his wedding unable to speak. In the hospital, House stabs him with a needle to see if he’s faking. He says, “Ow,” which is consistent with someone faking. However, he collapses again shortly after.

House, meanwhile, has been drinking more than usual. A worried Wilson arranges for House’s team to take him out to keep him busy.

When Taub and Thirteen break into Ted's old apartment, they meet Cotter who claims to have dated Ted for three years. Ted denies dating Cotter. Eventually he admits there was “contact” but still claims not to be gay. Thirteen later learns Ted went through gay conversion therapy including electroconvulsive therapy.

House has his dinner with Taub. House believes Taub is still having an affair. House brings Taub's wife to the dinner and leaves.

Ted faints.

Cotter turns up at the hospital while Nicole, Ted's fiancé, is sitting with him. He gets a bit too close to Ted for Nicole's comfort. Nicole asks Cotter to leave.

Nicole asks Ted about Cotter. Ted claims he had sex with Cotter once when drunk. He says he thought he was gay once but that he was cured. He suddenly gets a severe headache.

House goes to a lesbian bar with Thirteen.

Ted shows signs of a stroke. Houses insists on getting Taub, Cotter and Nicole together in the same room to get answers.

Nicole reveals that Ted sometimes has Erectile Dysfunction with her, which he never had with Cotter. Testing doesn’t find an issue with Ted’s penis.

House, Chase and Foreman sing karaoke and toast Wilson. Ted starts lactating.

Wilson starts talking about House having fun with Chase and Foreman and the difficulty with change. House has his "Eureka!" Moment.

House tells Ted he has a Chiari malformation. It didn't become a problem until his electroconvulsive therapy plugged the opening. Surgery will fix it.

After the surgery, Nicole calls off the marriage.

Taub calls off the affair. He thanks House for trying to help.

Cuddy asks House out to eat, but he declines. She wants them to be friends but that’s not what he wants.

House pours himself a drink.

This episode contains examples of the following tropes:

  • Cure Your Gays: What the patient attempted to do to himself, including hormones and electroconvulsive therapy.
  • Dramatic Irony: The patient claims to be “as straight as any of you”. Unknown to the patient but known to the audience, and all other characters present, is that among the “you” is the bisexual Thirteen.
  • "Eureka!" Moment: Talking with Wilson about change, leads House to the realisation of Ted’s congenital condition. It wasn’t until Ted tried to change himself with the electroconvulsive therapy that he managed to make it dangerous.
  • The Loins Sleep Tonight: Ted has this issue with Nicole but not with Cotter. Naturally this throws even more scepticism on his claims about the effectiveness of his conversion therapy.
  • Title Drop: While no one literally uses the words, "The Choice" Ted claims "I've made my choice" about wanting to be with Nicole rather than Cotter. Chase also says that while Thirteen currently dates both men and women, she will eventually need to make "A choice".
