The last chapter of "The First Year" is about the day Andy (Arnold and Helga's first child) was born—and apparently, Arnold had convinced his wife to have a vaginal birth instead of a c-section. While this may not seem like much, but of Arnold and Helga's three kids, Andy's the one who looks the most like Arnold...which includes having inherited Arnold's (in)famous head-shape. So this ultimately means that Helga's vaginal birth ends up being far more painful than it would've been even if Andy didn't inherit his dad's head shape—while the experience is clearly no fun for Helga, it is kind of funny to Helga so pissed at her husband, screaming her head off and giving him the whole "YOU DID THIS TO ME!"-speech (even grabbing by the collar of his shirt at one point) and then squeezing his hand hard enough to (literally) almost break it.
After Gerald and Phoebe (who was six months pregnant with her and Gerald's first child) arrived at the hospital a few hours after Helga went into labor with Andy, Gerald expresses worry for the couple—Stella (Arnold's mom and Helga's mother-in-law), logically assumes that Gerald must be expressing worry for Helga and assures her son's best friend that Helga will be just fine. But Gerald admits (to himself) that he's actually more worried about Arnold than Helga (given what Helga put Arnold through during the pregnancy and what she was currently putting him through in the delivery room, Gerald's not entirely wrong).
Just like with Danny and Sam from Danny Phantom, it's funny to see how Andy and Kiara vehemently deny having feelings for each other whenever someone implies that they do.
"Motherhood" shows that while Helga may have mellowed out a lot as she's gotten older (especially by the time she and Arnold have gotten married and had kids), but she's clearly Not a Morning Person and is grouchy until she has that first cup of coffee.
