I'm pretty sure that the creature in the prologue is just another one of the human skin ghosts. There's a direct call-back to it; Rick calls the monster "a creature" in the prologue, and he also refers to the ghost as "a creature" before realizing to his shock that he somehow never noticed that it didn't have legs.
