* Sankekur's letter to Lazarus talks about the latter's task of guarding Mephisto's Soulstone, and how it had taken a toll on Lazarus' sanity, and demands that they meet to discuss that task. Lazarus probably shattered Mephisto's Soulstone and stabbed Sankekur and the High Council with the shards himself.

* Marius is the fourth adventurer from the 1st Diablo because the game allows a maximum of 4 players.

* This would explain why he found refuge at the Rogue's monastery and can't sleep at night.
* It's no coincidence that he followed Diablo because he too was corrupted during his time in the dungeons.

* Nightmare is their... well, nightmares which they get from fighting demons all day.

** As Ormus says, "The Travincal can be breached by the loss of one's wits, not by the use of them." That's probably how Marius breached the Travincal: he was pretty much insane by that point. And as for why the demons and Zakarumites didn't stop him... he was no threat to them, and they knew it.

This explains all of the InexplicableTreasureChests everywhere.

How else could he slip into the third floor of the Durance of Hate unnoticed?
* The durance of hate was overrun by demons after Mephisto was released. The place was quite empty beforehand.

** One could easily assume that the wanderer cleared the floors as he made his way to the chamber. Through all the cut-scenes, Marius never showed any real measure of conpetence at anything.

Think about it, how would you feel if you were forced to kill Baal several hundred times for that Pelta Lunata or any other unique equipment?

That's why the characters don't hop the wall and cut over to the next zone. The areas behind the walls are just filled with poison ivy and poison oak.
