** The new designs are somehow even more uncanny than the original- the grey decaying skin, distorted faces, inhuman voices, and unnatural movements are all there, but these spirits often have something extra to really make them spooky. For instance, the Urashima Woman has an unnaturally enlarged stomach (from all the [[spoiler:fetuses she took in]]) and when you face her, she turns out to have [[spoiler:a giant LampreyMouth, with moving insides]]. Meanwhile, the Screaming Author [[spoiler:looks like an unholy cross between a little girl and a crane]]. The only aversion is the Demon Tsukuyomi, which looks downright alien.
** Kakuya, the BigBad, is a CreepyDoll who looks cute at first glance... until you see her face. It's plastic, fake-looking, with big eyes and small lips that look wrong, like they were [[DigitalHeadSwap Photoshopped on]]. Later on, we find out she has four arms, and the bottom ones are creepily long, and even more doll-like. [[spoiler:And then there's her final form, where she turns into some kind of butterfly-centaur-dryad hybrid with an asymmetrical face, half of which is grey and decayed with a hole where her eye should be and rotting gums.]]

Helped along by his childhood friend and an idol who was close to the first victim, alongside a variety of other colorful characters, Akira is forced to play Kakuya's twisted game.
