I can see the argument against the current (and I supported it in the old thread), but I think it's a Keep Until Better Image Suggested situation. That said, I do have two suggestions, from Maken-ki! and the "Shinji falls on Rei" scene from Neon Genesis Evangelion.
I don't get the left one, right one would work better if the girl is shown to be appalled. The unfortunate character cannot be considered a pervert, if it isn't confirmed in-universe. Or am I reading the trope too narrow?
Well, the problem with the NGE scene is that while Shinji is really embarrassed, Rei is completely neutral. It's going more for the visual than complete adherence to the trope. As to the first, I just realized that it's not flipped, and I don't have the skills to do that and preserve the characters, so that's pretty much out.

It's more fitting for the subtrope Suggestive Collision though.
Not a fan of the NGE one because Rei's state of dress is barely noticeable.

If a tree falls in the forest and nobody remembers it, who else will you have ice cream with?I figured this trope comes in two flavors:
- A physical one, involving a victim who feels sexually assaulted by a person who didn't mean any harm.
- Associated tropes are Thanks for the Mammary, Suggestive Collision accidental Clothing Damage, etc.
- Associated images are 2.1., 2.2, 7.1 and 7.4
- A visual one, without a victim, where an onlooker (in-universe) misinterprets a scene and falsely labels a character as pervert.
- Associated tropes are Not What It Looks Like, accidental The Peeping Tom.
- Associated images are 7.2 and 7.3
And of course there are cross-overs of both flavors.

I don't think it's mutually exclusive with Not What It Looks Like, as that trope can apply to many different situations that are taken out of context, including non-sexual ones. The page image has what looks like animal abuse at first sight, for instance. I think an in-universe reaction is an improvement though, so I consider 7.2 or 7.3. improvements.

You've got roaming bands of armed, aggressive, tyrannical plumbers coming to your door, saying "Use our service, or else!"^ You are right, there is a an overlap which I haven't considered first. A Not What It Looks Like situation with sexual content falls most likely also into Accidental Pervert.
IMO, in-universe is not a nice-to-have but a requirement here. Out of universe is Unfortunate Implications as in the current image. The work is innocent but the creator didn't consider the audience' perception. May lead to misuse.
Accidental Pervert really needs to be called out on it for it to really be Not What It Looks Like.
An image really should show a 'You pervert' and/or 'apologizing and such'.
To Love RU has many perfect moments for it, especially between Yui and Rito, but really gotta find one not so suggestive.

Ménage à 3 has this strip that reaches over two pages, where the male character is trying to cushion the girl with a blanket but his panting from their previous exercise makes it looks like he is trying something else. The girl then calls him out on it. The act itself is not well portrayed though.
That image only shows the calling out and not the accidental perversion, which I think is even worse than the previous suggestions.
I was gonna suggest the scene in Back To The Future when Marty's back in time and ends up in his mother's bed in his underwear and she doesn't seem phased that she took off his pants while he reacts uncomfortably, but I can't find a good picture example

^ As discussed above, Not What It Looks Like can overlap with Accidental Pervert if a non-sexual situation is mistaken for an offending sexual act.
Except none of those are that, most of those are sight gags with phallus shaped objects.
A true accidental pervert well needs to be clearly accidental and clearly perverted.

That images is depicting an Unfortunate Implications moment, where the creator did not consider the audience. Accidental Pervert is when somebody unintenionally offends a character in-universe. Like when the girl in a Thanks for the Mammary or Suggestive Collision situation accuses the male character of being a pervert.
For the current image we would expect to see the girl calling out the boy for his accidental hands-on approach. But it's only us the audience who draws the wrong conclusion.
Barring any upcoming suggestions, I vote for pulling.
