I'm not familar with Donkey Hodie, but two of the examples are book adaptations that don't accurately portray the episodes. Is it possible the books just ... changed things? If those are "mistakes", I have a lot of notes about the Doctor Who novelisations...

I pulled this from the Doctor Who section:

  • An article in The Daily Express, about advice Thirteenth Doctor actress Jodie Whittaker had received on playing the role, from previous actors who had portrayed the Doctor. The article went on to say that Patrick Troughton who had previously played the Second Doctor had advised Whittaker "not to stay in the role for more than three years". How this was possible considering Troughton had died in 1987, 30 years before Jodie Whittaker was announced as the Doctor.

It's a terrible article in several ways, but it doesn't exactly say that. It says:

Whittaker considered the words of the late Patrick Troughton, the Second Doctor, who was cast in the role for three years until 1969, to not play the role for more than three years.

Which is true; those were his words. It could probably be clearer that they weren't his words to her, but to Davison, and possibly to future Doctors in general, but it doesn't actually claim otherwise, and does acknowledge he's dead.
