Single word titles:

Depending on how long the title is.
Replace the sample title with the title of the work.

You could also just add it to the YMMV page under Heartwarming Moments or Tear Jerker if it's only one or two examples.

Or add it to the YMMV page instead. If you create a page in the Heartwarming or Tear Jerker namespace, you'll have to enter the URL to the page since you can't select them from the drop down menu.
You would type in, as stated above, Tear Jerker/Heartwarming/etc. If no such page exists, there will be a message saying that, and an "Edit New" button in the top left corner. Click that and edit away
