I think that note means if the character received the name in-universe because of something they did or are, it doesn't count.
So if Luke had been born Luke Willis and became Skywalker because he was a great pilot, then it wouldn't count as that trope.

... So if a character was born as Olaf, and later in his backstory acquired "Skullcrusher" as a surname due to his reputation for crushing skulls of enemies, it doesn't count? That's pretty much the basis for examples such as Warhammer and Warhammer Forty Thousand's Orcs/Orks, since they don't have families and thus the concept of "family name" is alien to them, so an Orc's/Ork's surname is always an epithet acquired from due to some action/incident/personality trait/place/history/etc that's associated with the Orc/Ork in question.

Correct, there are many invalid examples under the definition we have for the trope.
This is a job for TRS to decide if Earned Epithet should be it's own trope or included in Luke Nounverber or if Luke Nounverber should just be cleaned up.

This passage from Luke Nounverber is confusing me:
Big problem: The Trope Namer himself would thusly not be an example, as "Skywalker" is stated in one subentry of the example to be referring to his Ace Pilot skills. Same goes for many/most of the listed examples.
Does this look like a job for TRS?
