I really like that dA pic, but something simpler like this◊ might be better. I'd also really prefer to not use a RL pic for this, both for squick avoidance and to keep people from confusion with Eye Scream.

Also, the Bread, Eggs, Milk, Squick joke doesn't work if the procedure in itself is squicky or close to it to begin with.

Lobotomy is a new trope and could use a picture. I did a google search and must say that most of them freak me out.
Nevertheless, I have two suggestions.
1.1 This one is from DeviantArt: Transorbital Lobotomy. I like it as a whole, though perhaps we could use just a part of it.
