Under the Knife

  • Derek and Angie burying the hatchet at the end of episode 1-8.
    • In the Japanese version, the end of that episode's operation is the shift where Angie stops referring to Derek as "Tsukimori-san"note  and starts respectfully referring to him as "Tsukimori-sensei"note .
  • Derek choosing to join Caduceus over staying at his old hospital, because even though he loses the close patient-doctor relationships that come with working at a local facility, he believes that the potential to save far more lives as part of a major medical organization outweighs the drawbacks.
  • The end with Derek and Angie reading the letters from the children that they saved in the last chapter was a very sweet way to end the game.

Second Opinion

  • At the end of Chapter 6-4, after you save Derek:
    Angie: (teary) ...Thank you...Naomi. I'm sure Derek will get well...soon...
    Naomi: I was just doing my job; no need to thank me. ...But, why would you want to hide your tears from him? You think he doesn't already know? That man deserves to see how you feel.

Under the Knife 2

  • When Blackwell comes down with Post-Savato, Derek, who's been going through a Heroic BSoD since the end of Chapter 3, manages to bring himself out of despair, regain his Healing Touch, and cure Angie's father, both of them having regained confidence in each other.
  • While the duo were held captive by Delphi and locked in a cell after being told on no uncertain terms that the organisation fully intend to subject the two of them to a Fate Worse than Death, Derek declared that he will not allow harm to come to Angie, and that he will do everything in his power to protect her. A sentiment that she greatly appreciated.
  • The final scene with Derek and Angie during the epilogue when she finally decided to implicitly confess her feelings for Derek was terribly cute. Especially considering how rockily their relationship started off in Under the Knife and Second Opinion, the duo have really come a long way.

Trauma Team

  • Most of the cutscenes from the Orthopedics missions qualify, due to how much of a saint Hank is. Throughout his campaign, Hank goes out of his way to try and help the suicidally depressed young woman find a new passion for life with no real motivations other than to help her learn how beautiful life can be. He applies that same logic to helping the gunman who endangered the two of them, treating the seemingly inoperable tumors that drove him to the act for no reasons other than a belief in the inherent goodness of humanity, and that he deserved a chance to see life was wonderful too.
    • Also his reaction to learning Maria was able to successfully resuscitate Claire. Their talk afterwards is able to reignite his passion to continue fighting as Captain Eagle.
  • When Alyssa suddenly flatlines near the end of her surgery, CR-S01 practically begs her to stay alive. He even commends her afterward for pulling through while concluding the operation.
    CR-S01: Alyssa... please! You've got the whole world ahead of you! You're not alone! Teach me... what it means to live!
    • Mind you, this is the only time he actively addresses the patient on the table directly, and it's a sweet little girl who eventually becomes another character's reason to live.
  • Naomi hugging Alyssa at the end. With everything she had gone through in the series, she deserved the happy ending she earned.
  • The end of Naomi's third mission has her sitting down with Joseph Parker, who has just gone through the trauma of having to kill his wife in self-defense after her brain tumor made her attack her family in a desperate bid to save their souls. In a heartfelt conversation, Naomi promises Joseph that she'll find out what caused the tumor and ensure that his wife can be remembered as the good woman she was.
  • While every game before Trauma Team has a rating system, this game adds in a bit more flair to it with the Doctor and that episode's assistant commenting on how well they performed. Getting an XS rank especially feels like they're singing your praises. This includes the usually blunt and stoic CR-S01 and the feisty and impatient Maria. note
