The twist is that when the doctors and nurses, in-shadow from the very beginning of the episode, are finally revealed to have grotesque sagging faces with pig-like-noses, and the world that these beings live in has beauty standards that are completely reversed compared to human standards, and is also a dictatorship that shuns and persecutes people who are comparatively beautiful.
