Definitely the save point thing. I have no idea how many times I've wanted to stop but couldn't until I found a save point, but I know it's too many.
An NPC is being a dick to you in a game with quicksaves? Save, murder them, then reload the save, content with the fact that they're dead in a different timeline.
Got shot inexplicably behind a wall, even though you know for 100% certain you got behind cover in time? Got dropped by LESS than the required amount of bullets to kill a typical character? Death the very instant you turn a corner and see an enemy who is aware of your presence?
The Answer? Internet latency, especially if you live in areas where the connection is subpar. Probably also accounts for more than half of all of the "OMG HAX" complaints you'll see online.
Raiding an massive dungeon in an RPG or playing Lo L with the expectation that you have the next hour to yourself...And then getting assigned an random chore in the middle of an fight.

Also, in Batman: Arkham City, there's a sidequest involving destroying every security camera in the city with a batarang - sure enough while I was actively playing it, I started noticing real life security cams more often and feeling as though I needed to aim a batarang at them. Finally, my Player Tic with any Grand Theft Auto game from Grand Theft Auto III onward is walking on top of parked cars, and sure enough, for a while when I saw a row of parked cars in real life I started almost wanting to do the same thing.
Being The Medic in any online game is always hard. Most if not all folks eventually get bitter due to dealing with horrible or ignorant players overtime.

For me it's making a noise when making a hard jump in an action or platform game, especially when it's over a pit, and seeing little blocks every time I close my eyes after too many block-related games in one sitting.
And needing a save point before you can quit.
