I neglected a treasure chest in Dragon Quest 7. Said dungeon involved lowering water to be able to explore it. The chest I missed contained a puzzle piece needed to progress. You can't lower the water again once you finish the dungeon. I saved.

Half Life: Opposing Force. I thought one of the more useful weapons lying on the ground was an enemy (it helps it's a nasty alien creature).

Got myself stuck in a room with those stupid one-way ice blocks on the floor and no keys. Saved. Had to start over.
Happiness is zero-gee with a sinus cold....I just remembered my first playthrough of Ocarina Of Time. The game's really easy. But looooong.
After becoming an adult, I went to the water temple, I raised the water level and for some reason I left the game running while going to eat real fast. I came back to find myself dead. No biggie, I saved the game to continue on.
It's then that I realized that raising the water level makes it impossible to climb up to a tiny ledge at the bottom of the temple that lets me open a door I must go through to continue on.
It...wasn't fun. Even if the game was easy, it was still quite far into the game, and I didn't have enough time to beat the game before having to return it.

I gave it some thought and decided not to break it up; it'll be worthwhile to see what happens if I stay loyal.
But now after seeing how the romance with Garrus plays out online, and the fact that he and Shepard are essentially homies-for-life, I really regret not choosing him.
Shin Megami Tensei : Strange Journey, I accidentally shifted my alignment to lawful from neutral right after beating Ouroboros for the first time and my entire team was more or less neutral creatures and relied a lot on combo attacks. Least to say, round 2 of that fight continually wooped my butt until I decided to get smart, dump the rest of my team and grab a low level doppelganger and exploit the AI to make it attack with elements that the doppelganger could reflect. This resulted in the attack being reflected and the boss taking about one a fourth of its total HP in damage. Repeat and rinse and suddenly my Space Marine was ten levels higher afterward and I shifted back into neutrality and continued with my previous team.
Felt that entire strategy was ingenious on my part, using a weak demon and use its lack of defense and reflective capabilities to turn it into a mega cannon against one of the toughest fights in the game.

The thing about making witty signature lines is that it first needs to actually be witty.Fairly sure that if you miss it, the same shard appears in a different place.
Maybe, but you can *also* lose shards by having them be carried by the wrong person at the wrong time. Which happened to me, grr.

This mostly happens in Mass Effect and Mass Effect 2. Wanna know why? Because SOME GENIUS THOUGHT IT WOULD BE A GREAT IDEA FOR THE "X" BUTTON TO BOTH SKIP LINES IN A CONVERSATION AND CHOOSE ACTIONS/DIALOGUE CHOICES. YOU KNOW. ...Let us in...THIS...is the problem I have with autosaves in games these days.
Thankfully most games that have autosaves also have manual saves that you can load just incase you get screwed over.

When I first started playing I didn't have a laptop, so if I wanted to check the guide I'd have to run back and forth between my computer and my tv. Since I didn't care to write any of it down, if I forgot a detail it could be bad.
P 3 P fixed the first issue, and writing down the details Sparknotes-style did for the second, but I remember one time I was playing and got tripped up by thought-memory. I had to go to the oracle draw two weeks apart, for different links, and I had to have a weekend meeting with one in-between. So I draw the first oracle, have the meeting and forget that I need to draw for the OTHER S Link. This ended up screwing over the rest of my links as I tried to repair it, and I ended up missing out on Orpheus Telos.
Cue rage.
The other time that stands out in my mind was when I forgot to forge the Flail of Ages in Baldur's Gate 2. I had all three of the flail heads, but didn't know that I would be locked out of the forge unless I was a fighter, barbarian or monk. Guess what playthrough I had decided to play as a cleric?

My other signature is a Gundam.Suikoden. Need I say more? Yes? Fine. The 108 stars. Some of which have a point after which they'll stop being available. I've saved and lost so many stars because of the fact that there's no way to tell unless you consult a walkthrough.

Only some quests give the black thread...and they stop becoming available. Your inventory have so little space that you eventually have to discard an item in order to get a new item. For a person without any guides of FAQ's back then, this resulted in me dumping a black thread that I need to do another quest in order to get another item.(I had plenty of them back then, so I thought they weren't that important)
I had to start all over again.

Ever had that happen to you? Assuming you did it right in the middle of a long game. What did you do?
For example, you accidentally chose the option you didn't want just to see the dialogue...like you killed the Woobie or your favourite character. You deprived yourself of a character or something you really wanted. Or it's a game where you thought up a story in your head and fully intending the story to go a certain route. But thanks to Damn You, Muscle Memory!, you accidentally saved in an attempt to get things over with fast.
And most importantly...there is no way of undoing what you did...and it happened midway through a long game.
I'll start.
tl;dr: I was playing a heroic good guy team* . I chose the evil path. Accidentally saved.
Reaction? Start all over again.
