The South Park example - a lot of episodes, not just the earlier ones, have been given a TV-14 rating, without any syndication edits - only "f**k" is beeped. Even Death Camp of Tolerance aired with a TV-14-DLSV (Some episode like Ginger Kids and Le Petit Tourette were still TV-MA though).

Sesame Street no longer seems to have its own Nightmare Fuel page. Is the link just temporarily broken, or has it been removed?

It's when an entry in a movie/video game/manga/whatever series gets its rating marked up from other installments, even though the actual content hasn't changed much at all.
