The Russian Badger showed a game where certain player perks refund/negate the ammo cost of some weapons for a brief period of time, but they could stack to ridiculous levels...
Cue a M1 Garand firing 25 rounds non-stop; This weapon can hold 10 rounds. I'm not sure what was the name of the game; was it Insurgency or Payday?
Another famous example were abilities/perks on the Rogue of World Of Warcraft, where a critical strike would ensure the next strike would have the chance of also being a critical strike, increased by a fair percentage, like 20% or 25%. The problem was, the perk could go beyond 100% when the weapon or some gear also had that perk, which meant that every strike save the 1st was sure-fire a critical one, and generally a One-Hit Kill. And yes, it STACKED regardless if the previous hit critical percentage was due perk or random chance, which caused the problem.
This also applies to a number of games too large to mention, being the 1st completely ridiculous and the 2nd exceptionally famous.
