Another famous example were abilities/perks on the Rogue of WorldOfWarcraft, where a critical strike would ensure the next strike would have the chance of also being a critical strike, increased by a fair percentage, like 20% or 25%. The problem was, the perk could go beyond 100% when the weapon or some gear also had that perk, which meant that every strike save the 1st was sure-fire a critical one, and generally a OneHitKill.

And yes, it STACKED regardless if the previous hit critical percentage was due perk or random chance, which caused the problem.
