Unfortunately, this one does not occur for "no discernable (in-universe) reason". It is justified. Or at least I think an overload is a reasonable justification.

Specifically, a laser on a Powered Armor. Due to overload/overuse.
Could not find it searching the site or in the Iron Man pages.
Also, Reliably Unreliable Weapons does not seem to fit.
