The original Murphy's Law in action (designers should make it as difficult as possible to screw something up by accident, and failure to do so will inevitably cause problems.), and an aversion of Are You Sure You Want to Do That?

Is there a trope for when a program doesn't have an "are you sure? yes/no" type of prompt before doing something really important? Very much Truth in Television but is sometimes used in fiction, usually Played for Laughs by having the thing the machine is does be super important so it really should have such a feature (e.g if there was some computer in charge of nuclear command where you just clicked somewhere on the world map and pressed N to nuke the place, and there was no "are you sure you want to?" prompt. no way that could go badly right?)
